THE PRACTICE TEAM

Practice Nurses

Tracy Hayes-Bradley (RGN)

The practice nurses offer advice and screening to patients on health promotion.

Will see patients with minor injuries, minor illnesses, run child vaccination clinics; provide cervical smears, travel vaccinations, contraceptive injections and offer reviews for patients with long term conditions such as asthma and diabetes, COPD as well as a number of other services.

Health Care Assistants

Gemma Breedon (NVQ3)

Our HCA works with the practice nurse. Duties include taking bloods, well man and well woman checks, NHS Health Checks, blood pressures, suture removal and dressings, smoking cessation advisor, diet advice and Flu Clinic.

Practice Manager

Mandy Brown (Dip Primary and Healthcare Management) is the Practice Manager. She is responsible for the overall management of the practice. She will be pleased to discuss any non-medical problems with you and welcomes any suggestions you may have for improving the service provided. She is responsible for the practice complaints procedure.

The Reception Team

Lynne Flavin, (Senior Receptionist). Natasha Garner. All information they receive will be treated in the strictest confidence. Registration of new patients and any information or documentation required for outside agencies. General office and reception duties e.g booking of appointments, arranging child surveillance and child vaccination appointments, ante natal enquiries etc.

THE HEALTH CENTRE TEAM

Health Visitors

A health visitor is concerned with the health of the whole community, offering particular attention to those with special needs, children and the elderly.

Health visitors run their own clinics and support parents and carers with problems which arise as a result of bringing up children.

Information can also be supplied to clients on ways of maintaining and improving health, both individually and to groups.

District Nurses

They provide skilled nursing care to our clients at home and can also offer support and advice on general health issues.

Community Child Health Nurse

This nurse provides pro-active health surveillance and health education in the Joseph Whitaker family of schools and clinics in Rainworth Health Centre, including continence treatment as well as Hill View Surgery – Baby Clinic

Speech Therapy

Services are available through a doctor’s referral.

Counsellor

Services are available through a doctor’s referral

Midwife

The midwife provides care for expectant mothers and works with the doctors during antenatal clinics. An antenatal clinic is held by the midwife on Tuesday Afternoon. Home deliveries may be possible in suitable cases.

Physiotherapy

You will need a referral from your Doctor to see the physiotherapist.

There is a full appointment system with the following options available to you:

1. Routine appointments with a Doctor within 1 week

2. Appointments for long-term conditions can be booked up to 12 weeks in advance.

Please remember that one appointment is for one patient even if it is to discuss another patient. One appointment is for one problem. If you have more than one problem please make a double appointment. Please cancel your appointment if you can’t attend so that someone else may take your place.

You will be able to sign up to book appointments online. To register bring photo ID to the practice and sign up at reception.

You will be able to register for our text messaging service. You will receive reminders for appointments the day before and we may also text you about other health related matters from time to time. Ask at reception or when on the phone to register.

HOME VISITS

Home visits need to be requested before 10am. Home visits are available for those too ill to come to the surgery. Please come to the Surgery if at all possible. This ensures the doctor has all facilities at hand and that the doctor’s time is used effectively. You will also be seen sooner.

On average a doctor can see four patients in the health centre in the time it takes to make one single home visit. We always try to see sick children the same day. Lack of transport is not a valid reason for a home visit; the doctor is not a taxi service!

Same day appointment to be booked from 8.00am - 9.00am

Emergency visits, for life-threatening cases, can be requested at any time. These requests may be put through to the doctor on duty who will visit at the appropriate time according to need. Sometimes, for example in collapse with unconsciousness or severe chest pain, it is safer to call the ambulance first, before the doctor.

For health advice and information you can call NHS Direct on 0845 4647 or access their website at Their services are freely available 24 hours a day, 365 days a year.

OUT-OF-HOURS ARRANGEMENTS

Our doctors cover from 8.00am to 6.00pm weekdays. Outside these hours we pay for an emergency deputising service using GPs who practise elsewhere in the Mansfield area. NHSNottinghamshireCounty are responsible for providing these services and their contact details can be found at the back of this booklet. Please avoid using this service if possible - we would rather see you during normal hours.

REPEAT PRESCRIPTIONS

There are 3 ways you can order your repeat prescriptions:

1 Bring in your prescription slip. Please circle the items you require and leave it in the box provided in the waiting area.

2 Telephone 01623 795562 between 12 – 2pm

3. *****Coming Soon***** Order online. You need to register for our online service by bringing photo ID to the practice and signing up at reception.

Always order 2 working days in advance and remember that items ordered on Friday will not be ready for collection until Tuesday evening. If you send a stamped, self-addressed envelope we will post your script to you. If you are elderly and have difficulty coming to surgery the local chemists operate a collection and delivery service. Telephone your local pharmacist for further information. Please do not request repeat prescriptions during consultations.

RESULTS OR TELEPHONE ADVICE

You may telephone for results at any time and the receptionists will try and put you through to either the practice nurse or health care assistant. If they have a patient with them, you may be asked to leave a contact number and they will ring you back when it’s convenient. The receptionists may in certain circumstances pass on comments from the GPs about your results but cannot discuss them further or give out more detail.

ACCESS FOR DISABLED PATIENTS

We will offer full facilities for the disabled including a hearing loop for patients with hearing difficulties. All staff will be happy to assist you.

COMPLAINTS

If you have a complaint or a suggestion to make, please address it to the practice manager. It will be investigated promptly with an appropriate response issued in a timely manner. We welcome your comments and will make every effort to ensure that problems you experience are quickly resolved. You can get a copy of the practice complaints procedure and a complaint form at reception or by downloading it from the practice website.

You may also find it helpful to speak to the Patient Advice and Liaison Service (PALS) who may be able to give you advice or help resolve any problems you may have. You can contact them on 0800 028 3693.

CONFIDENTIALITY

We ask you for personal information so that you can receive appropriate care and treatment. This information is recorded on computer and we are registered under the Data Protection Act.

The practice will ensure that patient confidentiality is maintained at all times by all members of the practice team. However, for the effective functioning of a multi-disciplinary team it is sometimes necessary that medical information about you is shared between members of the practice team.

ZERO TOLERANCE

We strongly support the NHS policy on zero tolerance. Anyone attending the surgery who abuses the GPs, staff or other patients be it verbally, physically or in any threatening manner whatsoever, will risk removal from the practice list. In extreme cases we may summon the police to remove offenders from the practice premises.

HOW TO REGISTER AS A PATIENT

2. Via the Internet; complete a registration form on the surgery website and print off the other forms that need completing.

PREFERRED PRACTIONER

You are entitled to see you preferred GP for routine appointments. However, this will not be possible for urgent or emergency appointments that must be seen that day. You may also find that you have to wait longer than usual if you only want to see your preferred GP.

When you register with us you will not be assigned to a particular GP, though that doesn’t stop you from expressing a preference to see a different GP when you book appointments.

SERVICES OFFERED

Minor Surgery Clinic

Dr Jairam offers a minor surgery clinic.

Minor Injuries

Minor injuries such as sprains, bumps and minor cuts can be seen at the practice. Please call and ask for an appointment.

Family Planning

We provide advice on all aspects of birth control during surgery consultations. Please make an appointment to see a nurse for contraception advice.

Dr Jairam fit coils and Implanons. Please make an appointment with the GP to discuss these options.

Chronic Disease Management

This includes diabetes, asthma, COPD, heart disease and stroke. Patients will be invited for an annual review by the Practice Nurse but can make an appointment with the Practice Nurse between these times if they have any concerns about their condition.

Well Woman Clinic/Well Man Clinic

Well woman and well man health checks are available with our health care assistant. Ask at reception for details.

Vaccinations

Every autumn we have flu vaccine clinics. These are mainly for the elderly (over 65), immuno-compromised diabetics, those with chest problems (eg asthma and bronchitis), heart problems (eg angina), kidney problems and pregnant ladies.

There is also a vaccine called Pneumovax II available at any time against bacterial chest infections and pneumonia recommended for the same people.

Smoking Cessation

Smoking cessation appointments are available with our Health Care Assistants to help you quit.

Private Certificates and Letters

A self-certificate SC1 is normally sufficient for the first week off work. Private fit for work notes are available from day three to day seven of an illness. There is a charge for these. Letters to appeal against Disability Living Allowance ratings, to the Council, Citizens Advice Bureau and Housing Associations also attract a nominal fee.

MINOR ILLNESSES

Coughs, Colds and Sore Throats

These illnesses are extremely common and troublesome. Most of these infections are caused by viruses, which will not get better with antibiotics. Even if the illness is caused by bacteria, it is not always necessary to treat with antibiotics as the human body is capable of fighting these infections naturally and there is little evidence that antibiotic use in these infections will make you better any quicker.

Overuse of antibiotics leads to the development of super-bugs (e.g. MRSA). It is better to only use antibiotics when they are really essential.

There are certain times of year when these infections are more prevalent. If you have had symptoms for more than five or six days and have tried various preparations from the chemist without result, then feel free to see a GP or nurse for an assessment.

Minor Accidents

For simple cuts and grazes, wash the wound under running water and apply a clean dressing or plaster; available in all chemists and supermarkets. If the wound does not stop bleeding after 5 minutes of pressure and it’s during surgery hours you can be seen by either a Practice Nurse or a GP as an emergency to review the injury. Treatment will be offered as appropriate for any minor injuries or concerns.

Any accident or injury out of surgery time must be seen at the hospital.

Diarrhoea and Vomiting

Most cases of diarrhoea and vomiting are caused by airborne viral infections, although some are passed by direct contact. They are usually self-limiting.

Symptoms include diarrhoea, vomiting, abdominal cramps and fever. Babies with diarrhoea and vomiting are often well in themselves - but if you have any uncertainty then, please, see a GP for assessment. Signs of dehydration include sunken soft-spot, dry mouth, dry nappies and change in colour of skin. If any of these signs occur then see a GP as an emergency.

It is usually not recommended to change your baby's feeding regime when they have diarrhoea and vomiting. In the past we used to advise watering down feeds, but this is not necessary. Just offer usual fluids and milk/feeds.

If you have any doubt at all about your child/family member, then do not hesitate to come for an assessment. If you think you may have food poisoning then bring a faeces sample so it can be sent for analysis.

Paediatric Surveillance

Eight week checks are performed by GPs. Baby Clinic every Wednesday 11.30am -12.30pm Baby vaccinations are also given at this time or alternative times are available if required.

Fever Symptoms in Children

There are lots of illnesses in children that can cause fever. Most of the time they are simple viral infections. Below are some simple measures for controlling a fever, when to consult a GP about a fever and any warning signs you should look out for.

  • It is very helpful if you can keep a thermometer at home for monitoring temperature accurately. The forehead strip thermometers are not always very helpful. Chat to your chemist aboutthis.
  • If you have a child under three months with a fever, please seek medical attention as soon as possible.

If you have a child with a fever, who is otherwise well in him/herself (eg, eating ok, drinking well, alert, no rashes) then use paracetamol/calpol suspension as directed on the bottle, and encourage fluids. If he/she is no better after 48 hours then please see a GP for assessment.

  • If your child is unwell with a fever - more drowsy than usual, has a rash, isn't taking fluids very well, or if you are at all worried, then see a GP for urgent assessment. It usually takes us only a few minutes to assess your child, enabling us to offer appropriate treatment and advice, and we should be able to put your mind at ease about your child's illness.

Sun Safe Advice

  • Do not sit or work outside in the midday sun if you can help it.
  • Always apply a high factor sun screen at least 30 minutes before going out in the sun.
  • You can still burn even if it is a cloudy day
  • Beware of sunny, windy weather as you may not notice that you are burning.
  • Always wear a hat or sunglasses as eyesight can be damaged by UV rays.
  • It is better to be pale and free of skin cancer! There are lots of good fake tan products out there!

When should I call 999?

There are always reports in the papers about inappropriate reasons for calling for emergency help - 'I couldn't find my glasses' being one of the many inappropriate requests 999 have heard over the years.

The following symptoms or conditions are potentially life threatening and warrant a 999 call.
