Opening times
The practice is open Monday, Tuesday, Thursday and Friday 8.00 a.m. to
6.30 p.m. Wednesdays 8.00 a.m. to 8.00 p.m. Saturdays 8.00 a.m. to 11.15 a.m. Surgery times vary from day to day but in general there is a GP in surgery from 8.30 a.m. to 11.30 a.m. and 2.30 p.m. to 6.30 p.m. daily. Practice Nurses are in surgery from 8.30 a.m. to 12.30 p.m. and 1.30 p.m. to 6.30 p.m. daily. Wednesday 6.30 p.m. to 8.00 p.m. and Saturday 8.00 a.m. to 11.15 a.m. are for booked GP appointments only. Emergencies will still be seen by Northampton Out of Hours.The practice will not open on Saturdays at Bank Holiday weekends.
Making an appointment
To book an appointment telephone 01604 644126. You may book with any doctor, not only the one with whom you are registered. If you are unable to keep your appointment, please cancel it by telephoning the surgery so another patient may use the appointment. Please try to avoid booking non urgent or follow up appointments on Monday which is our busiest day. You may book appointments up to 3 weeks in advance and we also have some appointments, for booking 48 hours in advance. You may be asked what the appointment is for so you will be seen by the most appropriate person.
You can now book appointments on line if you are set up for SystmOnline. Our nurses have daily triage appointments for patients with an urgent problem. You will be asked to come to the surgery where a nurse will see you. If she is unable to deal with your problem she will be able to ask a doctor to see you.
Telephone access to GPs and nurses
Patients are able to speak to GPs and nurses at various times during the day if necessary, but may be asked to call back or may be asked to leave a number so that the doctor or nurse may call them back when they are not in surgery.
Home Visits
These may be booked for those unable to travel to the surgery due to illness or infirmity. Requests for visits should be made as early as possible in the day and preferably by 11.00 a.m. A GP May telephone before visiting.
When the surgery is closed
If you have an urgent need to see a doctor when the surgery is closed, telephone NHS 111. Only use this service if your problem cannot wait until the surgery is open. Remember that the out of hours doctor does not have access to your full medical record. If you have a life threatening medical emergency, please dial 999.
NHS 111 makes it easier for you to access local health services. You will be assessed, given advice and directed to the local service that can help you best. That could be A & E, and Urgent Care Centre or Minor Injuries unit, an Out of Hours GP, emergency dentist or a late opening pharmacist.

Our phlebotomists hold blood taking clinics from 8.20 a.m. to 11.00 a.m. on most mornings.
You may make an appointment with a Practice Nurse for dressings, immunisations, blood pressure checks and cervical smears as well as advice on healthy eating and stopping smoking. They also hold asthma and diabetic clinics. Our nurses also run triage sessions during surgery times. When you book a nurse appointment you will be asked what your appointment is for so that we can book an appointment of the appropriate length.
District Nurses
Betty McGowan, Anne Omenya and Julie McDonnell, are our District Nurses.
Our district nursing team provide a wide range of nursing care aimed at those unable to attend surgery, on a short or long term basis. To contact our district nurses, please leave a message with reception on 01604 790108.
Health Visitor
Our health visitorMandy Bentley may be contacted on 01604 670427. Mandy performs child development checks and immunisations and has details of local playgroups etc. Jayne Nicholson works with Mandy.
Midwife
Our midwife holds a weekly clinic for newly pregnant ladies. Book an appointment with her at reception when you are about eight weeks pregnant and you will be given a “booking pack” by the receptionist which contains a timetable for you to follow during your pregnancy. During her routine clinics she will be happy to discuss all aspects of your pregnancy throughout the antenatal period.
Counsellor
Marilyn Alexander provides a counselling service at the surgery on referral from a doctor or nurse. We are also able to offer referral to the Well Being service.

Repeat Prescriptions
When you receive a prescription which the doctor has given on a regular
basis, a repeat request will be attached. Please retain it and deliver or post it
to the surgery allowing 48 working hours before collection. You can also send
your repeat request to the surgery electronically. Please contact reception for
details. You can also fax your request on 01604 670827. We are unable to take
requests for repeat prescriptions by telephone apart from in exceptional
circumstances.
Dispensing
We dispense medicines to patients living in a rural village more than one mile from a pharmacy. Payment for medication can be made by cash or cheque only. The law does not allow us to dispense for patients living in Moulton. If dispensary is closed you may obtain medication from Rochmills chemist or Boots at Weston Favell. For details of out of hours pharmacies contact NHS 111. Several local pharmacies, including Rochmills will collect prescriptions from us and deliver the medication to your home. Several GPs and staff members of this practice are shareholders in Nene Healthcare who are part of Northamptonshire Healthcare Alliance who operate NHS Pharmacy. Each shareholder’s financial interest in less than 0.1%
Hospital Tests
Specimens for transport to the hospital laboratory must be delivered to the surgery by 11.00 a.m.
New Patients
We welcome new patients living within the practice area. When you register as a new patient you will be asked to complete a new patient questionnaire, which completes your registration with us. You will be asked to show identification.
Change of Personal Details
If you change your name, address or telephone number, please let us know as soon as possible. We will need to see documentation supporting name or address changes. If you move out of the practice area you will be asked to register at another practice
Antenatal and Postnatal care is provided by all the doctors in cooperation with the midwife.
Child Health Surveillance is undertaken by Dr Ireland and our health visitor. This service assesses the development and wellbeing of children less than five years of age.
Immunisations in children
These are carried out by the practice nurses in clinics held on Monday afternoons. Appointments are arranged by Child Health Services. If you need to cancel a child’s immunisation appointment please call 01640 644126.
Baby Clinics
The health visitor has details of the local clinics.
Diabetes clinics are held in the surgery on a monthly basis and patients with diabetes are sent appointments for these clinics.
Asthma clinics, run by a Doctor and nurse are held in the surgery. Contact reception for an appointment.
Smoking cessation
If you would like help to give up smoking, please ask at reception for an appointment with a practice nurse. You will be offered regular support and if appropriate, smoking cessation aids.
Support for carers
If you are the carer for someone, please advise reception so that we can note this on your records and offer support where appropriate.
Emergency Contraception
We are able to offer confidential advice and treatment for emergency contraception. If you think you require emergency contraception you need to seek advice as soon as possible, but no later than 72 hours after unprotected intercourse. This will usually mean that you can discuss this during normal surgery hours. You may also attend a pharmacy of your choice to obtain emergency contraception. A charge will be made if you attend a pharmacy.
Results
Please telephone the surgery between 11.00 a.m. and 5.00 p.m. for the results of any investigations. Most blood results are back at the practice within 5 working days.
Travel immunisation and advice
Please contact us as soon as you know where and when you are travelling, ideally 8 weeks before travelling. If you do not give us sufficient notice your vaccinations may not have time to become effective. We will need details of your date of departure, where you are going and what sort of accommodation you are staying in. You will then be given an appointment with a practice nurse so that we can give you general travel advice and advice on what immunisations, if any, you need. We do not provide those immunisations that are not available on the NHS. You will be advised to attend a private clinic. Complaints and suggestions
If you have any concerns or suggestions as to how we may improve the service we offer, or you wish to make a complaint, please bring it to the attention of the Practice Manager, Sue Saunders, who may be contacted by telephone on 01604 790108 or by letter. We would also appreciate feedback when you feel we have done something well. If you wish to make a complaint but do not wish to speak to the surgery first, please contact the Primary Care Trust on 01604 651100.
Advice and Information Service
This service can provide you with confidential advice and assistance in resolving problems or concerns with your local services. They can also provide information on health related matters and, if necessary, explain the complaints procedure. They can be contacted on 0800 587 0879

Local Involvement Networks aim to give members of the public a stronger voice in how their health and social care services are delivered. Run by local individuals and groups and independently supported – the role of LINks is to find out what people want, monitor local services and to use their powers to hold them to account. Private medical examinations
These can be arranged by appointment for insurance purposes. Pre-employment and HGV medicals are also available. Please contact reception to book a medical and to enquire about the cost. Payment should be made at the time of the examination. Missed appointments will be invoiced.
Other private work
The completion of many forms and letters normally attracts a fee as the work is not regarded as NHS work. Please ask at reception for the cost.
Sick Notes
These are not normally required unless you have been off work for a week. If
your employer requires a certificate for less than a week, we will be happy to
provide one privately. A charge is made for this service.
Dental Problems
Neither the Practice nor The Out of Hours doctors are able to provide consultations
for dentalproblems. There are two dental access centres in Northamptonshire
which provide urgent care for people who do not have a regular dentist. Call 01604
762217(Northampton) or 01536 403400(Corby) as close to 8.30 a.m. as possible
for asame day appointment. To find which dentists are now accepting patients, call
The Advice and Information Service on 0800 587 0879. If you need urgent dental
care whendental surgeries are closed, contact Shams Moopen’sDental Practice in
Northampton on 077 7044 5650between 9.00 a.m. and 4.00 pm at weekends and
on bank holidays and 6.30 p.m.and 10 p.m. in the evening. The evening service is
for telephone advice only. Alternatively you can call 111 and you will be directed to
an emergency dentist.
Disabled Access.
There is a designated parking space for patients with a disability at the front of the building. Entry into the building is then through the main door which is on the same level as the parking space. The door may be opened by push button control. If you need additional help please contact reception for assistance. A hearing loop is installed in the waiting area and portable hearing loops are available for use during your consultation. Please ask at reception or mention it when you are called in for your appointment.
Car Parking
We have a car park at the surgery, but space is very limited and we would therefore ask anyone attending the surgery who is able to walk to do so. We would also ask that everyone parks considerately. Please do not use the car park at Chappell House and be considerate to our neighbours when parking your car.
Medical Students
We sometimes have medical students visiting the practice, but patients will be advised of this at the time of their appointment and may choose not to have them present during their consultation.
New Patients
We are happy to register new patients living in our practice area. To register with us you will need to bring your medical card with you or alternatively you will need to fill in a registration form. You will also be asked to complete a new patient questionnaire and if you are in our dispensing area you will need to complete a form for this too. You will also be asked for 2 forms of identification, one of which should be photographic and the other should show your address in our practice area. If you are from overseas you will be asked to show your passport and visa. You may be asked to attend an appointment with a nurse if you have a long term condition or if you take regular medication.
Your rights and responsibilities
As part of our service to you we always try to treat you courteously and we try to offer appointments that are convenient for you. In return we ask that you treat us with courtesy and keep appointments made for you, or cancel them so another patient may use the appointment time. We do not tolerate violence or abusiveness towards our staff and will not hesitate to call the police if violence occurs. Patients acting in this manner will be immediately removed from the practice list.
Data protection
We regard the information we hold about our patients as confidential whether it is held in paper or electronic format. This information is not disclosed to a third party without consent from the patient. Test results and details about appointments are also considered to be confidential and will only be given to the patient unless the patient is under 16 years of age when they may be given to the responsible adult. All Patients have the right to access their own medical records, but this must be requested in writing to the practice manager. An appointment is then made with a GP so that the patient is able to ask questions if necessary.
Staff Training 2013
The Surgery will be closed for staff training from12.30 p.m. on6 February, 13 March, 17 April, 15 May, 12 June, 10 July, 11 September, 9 October, 13 November and 11 December.

Carers
If someone depends on you for their care whether it is a friend or family member then you are a carer. You probably see this as part of your life or your duty to care for them, but there may be times when you need information, advice or some extra help. Please let us know by asking at reception for a Carers Identification and Referral form, or pick a leaflet from reception or see the posters in surgery.
