WE ARE PLEASED TO INVITE YOU TO APPLY FOR OUR PUMA PRIDE SCHOLAR PROGRAM. THE PUMA PRIDE SCHOLAR PRORAM IS DESIGNED TO BE A HIGHLY RIGOROUS FOUR-YEAR COURSE OF STUDY. IF OFFERS A SELECT GROUP OF STUDENTS (60) A COMPREHENSIVE CURRICULUM COMBINING ACCELERATED HONORS AND ADVANCE PLACEMENT-LEVEL COURSEWORK.

It is the student’s responsibility to see that all parts of Forms A and B are completed and submitted as stated below (please see next page for a more specific timeline).
Student Responsibilities
Your application is to be completed by you and sent by mail, or hand delivery to Wise High School (attn: Ms. Natoya Coleman, PRIDE Program) by 3:30 p.m. on December 18, 2015. You should include Form A, Form B, copies of your final seventh grade report card and first and second quarter eighth grade report card and the required essay. Applications must be complete with NO MISSING COMPONENTS in order to be considered for review.
***Students will be notified via email on the status of their application, including application receipt and program acceptance.

Entrance Exam Information
· Students will receive an invitation to the entrance exam upon review of their application via EMAIL. Please ensure that the email address on the application is a working address.
· Students must bring 2 #2 pencils and a calculator to the entrance exam.
· Students must provide transportation to and from the entrance exam.
· Students must provide advance notification if they have a scheduling conflict. “No Shows” for the entrance exam will not be considered for admission.

Parent Permission Statement
I hereby submit this application for my student to be considered for the Puma PRIDE Scholar Program at Wise High School and give my permission to school officials to administer and report my child’s achievement and aptitude test scores, grades, and attendance records when required. I understand that this application and the test results will be held in confidence by all members of the PRIDE Admissions Committee and that applications will not be returned to students or parents.
I am aware that the Puma PRIDE Scholar Program is a Signature Program, not a Magnet Program, and therefore is designed only for students residing in the Wise High boundary area.
