6.1)NEED FOR THE STUDY :

Eye is a specialised sense organ serving the most vital function of providing sight to the living creatures.Vision is unarguably the most important of the five sense organs.Hence all sincere efforts should be made by men to protect his eyes,throughout his lifetime,as for a man who is blind ,the beauty of this world becomes useless.

Among the Ayurvedic Classics available today Sushrutha Samhitha gives a wide description of the diseases concerned with Shalakya tantra.Among them too Acharya Sushrutha has given more importance to ophthalmology,as it is evident that the Uttara tantra of Sushrutha Samhitha starts with Netra rogas and out of the 26 chapters of Shalakya Tantra, 19 chapters are devoted to netra rogas2.

When we see the classics, the symptoms of Timira are very much comparable with Simple Myopia, a common refractive error.

The global prevalence of refractive errors has been estimated from 800 million to 2,3 billion.The prevalence of myopia has been reported as high as 70-90% in some Asian countries with 41% of the adult population in India being Myopic to 1.00dpt3. Myopia is caused due to varied reasons like elongation of the posterior section of the eye balls and in certain cases degenerative changes in the eye causes myopia.Till date no medicine has been invented to preserve the sight of the patients affected with myopia and no surgical measures is fully safe from complications.Modern medicine offers the use of spectacles which neither cures nor prevents the progression of the pathology.

In Ayurvedic classics ,various treatment modalities like use of various chakshusya dravyas, nasya,ghritapana,anjana and most importantly “Kriya Kalpas” have been mentioned for the treatment of Netra Rogas includinf Timira.The Kriya Kalpa refers to the treatment which can be applied for almost all types of eye diseases and it comprises of Tarpana, Putapaka, Anjana, Aschyotana and Seka4.Acharya Vaghbata has mentioned that the netra bala should be achieved by tarpana etc and if not done so the varied beauty on the planet all look alike that is with a colour of darkness5.

Hence an effective treatment for Timira which is cost effective,simple and without any side effects is the need of the hour. Hence the present study is planned to find an effective treatment for the same.

Review of the literature includes thorough screening of classical and contemporary literatures, periodicals, e-journals etc.

6.3) Objectives Of The Study:

  1. To evaluate the efficacy of Jeevantyadi Ghrita tarpana in the management of Prathama Patalagat Vataja Timira.
  2. To evaluate the efficacy of Jeevantyadi Ghrita tarpana and Shatavari choorna internally in the management of Prathama Patalagat Vataja Timira.
  3. To compare the efficacy of both.
  4. To carry out intensive study in relation to aetio- pathogenesis of Timira and its management vis-à-vis Myopia.
  5. To derive a standard and easily accessible, cost effective treatment for this disorder.

7.MATERIALS AND METHODS :

(a) The patients:

The patients attending the O.P.D. and I.P.D. of the S.J.G. Ayurvedic Medical College & Hospital, Koppal will be randomly selected for the study.

(b) The Drugs:

The formulations with their respective ingredients will be prepared in the pharmacy section of S.J.G. Ayurvedic Medical College & Hospital, Koppal.

A detailed review of the available literature on the drug and disease, such as classical texts of Ayurveda, Nighantus and other compendium will be done.

Information will also be collected from various books on current surgery as well as herbal medicines, periodicals and papers published in the e-journal etc.

2) Experimental source:-

No animal experiment will be done. The patients attending the O.P.D. and I.P.D. of the S.J.G. Ayurvedic Medical College & Hospital, Koppal will be selected for the study.

7.2(c) PREPARATION OF MEDICINE:-

Medicine will be prepared as per classics in the pharmacy of S.J.G. Ayurvedic Medical College & Hospital, Koppal.

7.3METHOD OF COLLECTION OF DATA :-

7.3a) Sample, Size & Grouping:- It is a single blind pre-test and post- test design, where in minimum 30 patients of either sex fulfilling the inclusion criteria will be taken and put into two groups, with 15 patients in each group.

Group A: Akshitarpana with Jeevantyadi Ghrita along with Shatavari Choorna internally.

Group B: Akshitarpana with Jeevantyadi Ghrita.

Results will be compared at the end and statistically analysed.

7.3b) Inclusive criteria:-

  • Patients having signs and symptoms of Pratham Patalagata Timira.
  • Patients between the age group of 8-16 years.
  • Patients with refractive error from 1 to 5 Dioptre.

7.3c) Exclusive criteria

  • Patients below 8yrs and above 16 yrs.
  • Other than Pratham Patalagat Timira.
  • Spectacle users with more than 6D.

7.3d) Criteria of diagnosis:-

The symptomatology of Timira as mentioned in classical texts, in addition to the signs and symptoms mentioned in contemparory texts.

7.3(e) Posology:

Jeevantyadi Grita - q.s

Shatavari Choorna-12 gms11 in two divided doses.

Anupana: Dugdha

7.3 (f) Study duration:12 weeks

7.3 (g) Observation:

Group A:Tarpana done daily for the first seven days.This is followed by weekly observation.

Group B: Once a week.

7.3 (h) Follow Up:

Two Months.

7.3 (i) Assessment of results:-

Subjective and Objective parameters and baseline data to after treatment data will be compared for the assessment of result.

7.3(j) Subjective Parameter:-

  • Blurring of distant Vision.

7.3 (k) Objective Parameter:-

  • Snellen’s Chart.
  • Retinoscope.

7.3 (l) Investigations:

Routine Blood Investigation.

Any other investigation will be conducted as per requirement.
