The following documentation will be reviewed as evidence of compliance with the YP Inspection Criteria revised as of January 2015. Items 1-22 are maintained in a file box. Ensure the documentation box is standardized and organized according to the listing below. Only include documentation from the previous 12 months unless otherwise indicated below. In addition to the documentation box, the inspector(s) will review selected forms, youth files, employee work and training files. No documents with Personally Identifiable Information (PII) will be maintained in the file box.

Place the following items in the front of the inspection box:

  1. CYP Inspection Signs (to be displayed on the front door during the entire week of the inspection)
  2. Parent (to be displayed on the front door during the entire week of the inspection)
  3. Staff (to be displayed in the break or training room during the entire week of the inspection)
  4. YP Documentation List
  5. Blank copy of the program information worksheet (completed when the inspectors arrive)
