Library E-Rate Checklist

Submit Letter of Agency to SCLS (link to template below). Areas highlighted in red should be changed. Most of the changes are just to insert the name of the library or library system (those with multiple locations which would be Madison and Portage County).

Create an administrative account with UPC.
- Call 1-888-203-8100 and following the prompts until you get to an operator. Ask to have an administrative account set up for your library. You will need to provide them with the library name, the entity number (supplied by Vicki), your name, your email which will be your user name for the account.
- They will send you an email with the case and provide instructions for logging in to the account.
- Let Vicki know your account is created.
- Once you have access, correct any incorrect information in the account.

Create CIPA Compliant Internet Safety Policy
- Start with your current policy and update it to be CIPA compliant
- See CIPA Compliance section of E-rate FAQ for specifications
- Under CIPA, a minor is anyone "who has not attained the age of 17." Adults, therefore include everyone 17 years-old and older.

Board review of CIPA Compliant Internet Safety Policy
Find filtering solution for PCs not on SCLS network (SCLS will provide suggestions)

Provide reasonable public notice and hold at least one public hearing or meeting to address a proposed technology protection measure (filtering) and Internet Safety Policy (can be done at a regularly-scheduled library board meeting)
Trial of iBoss filtering on patron PCs (technology protection measure)

Filtering must be in place on all PCs and wireless

*Signed MOU committing to CIPA compliance each E-rate grant cycle must be submitted to SCLS
Form 479 must be submitted to SCLS; tips for completing it:
- It appears that the"Library" can be the "Administrative Authority" and the director can sign it
- You can make up the "Form Identifier"
- The Funding Year is 2017/2018
- You should check 6a only (and not 6b-6d)
