The Client Advocate Hispanic Services is responsible for medical support for the clients and the entire family and ensuring medical compliance for HIV/AIDS care. The Client Advocate will take the lead in coordinating care, finding new resources, and assisting with medical interpretation at medical appointments for the Hispanic community. Additional duties include small group HIV disease and medication education, client support groups, and research updated HIV medical information to provide to the Hispanic clients.
The Client Advocate reports to the Hispanic Latino Program Case Manager.
DUTIES AND RESPONSIBILITIES
•Work collaboratively with medical facilities to coordinate culturally confident medical care and follow up.
•Provide translation at medical appointments as needed.
•Develop and maintain positive relationship within the Hispanic Provider Community that allows the development of trusting relationships with target client base, (high risk for HIV+ transmission).
•Gather relevant health education materials for distribution to clients
•Assist with the development of audiovisual materials that are simple and visually understandable for clients to take home on basic medical adherence issues.
•Provide small group health education on a variety of topics connected to STDs and myths and realities in the treatment of HIV-related illnesses.
•Connect outreach to clients with all needed services in the areas of health, legal, educational, financial, and housing.
•Assist case manager by taking on high-intensity client assignments. .
•Document referral, education, case management work in AIRS ROSIE and ETO database.
•Document changes in the client’s medical condition, (CD4 Count and Viral Load), in accordance to grant requirements.
•Participate in regular staff meetings and case conferences.
•Adhere to basic confidentiality standard for AIRS clients
•Attend community meetings as appropriate, including health fairs and other outreach efforts.
•Other duties as assigned by the Case Manager of the Hispanic/Latino program
POSITION REQUIREMENTS:
•Minimum BA in healthcare/human service field with 2 years’ experience working with underserved populations.
•Solid understanding of HIV/ AIDS and STD issues, especially as it relates to the Hispanic Community in Baltimore.
•Must be bi-lingual, (English and Spanish), have good written and verbal communication skills, as well as computer literacy.
•Effective time management skills.
•Must have access to reliable transportation.
•Flexibility, creativity and enthusiasm for the project and for team work.
•Judgment when to seek Case Managers’ guidance
•Prior directly related experience may be substituted for education.
