**MUST BE COMPLETED, ADMINISTERED AND RESULTS READ, 7 DAYS BEFORE THE SECOND TEST WHICH IS SCHEDULED FOR JUNE 15** / No later than June 5
  1. The second must be administered by Enloe Medical Center
  • This will be administered June 15 at 1pm
  • Results read – June 17 at 1pm
**We will go over to Enloe as a group. You must have a parent signature on file before you can have the TB Test administered.** / June 15

COMPLETE ENLOE HOSPITAL’S O.A.K. SAFETY TRAINING MODULES
  • You will be receiving an e-mail from Kim Woodhouse – Enloe Recruiting office
  • You will need to complete and pass ALL Safety Training Test Modules

OBTAIN IMMUNIZATION RECORDS
Can be obtained from your pediatrician or your school
  • MMR: (2 shots OR proof of positive Titer)
  • Tdap (Tetanus, diphtheria, acellular pertussis) 1 injection since 2010
  • Varicella (chickenpox) - (2 shots OR proof of positive Titer)
  • OPTIONAL: Hepatitis B (3 Shots OR proof of positive Titer)

TURN IN ENLOE HOSPITAL’S REQUIRED DOCUMENTS
  • Read over ALL Enloe Hospital documents
  • Sign, date and return to Ellese Mello on the first day of class

  3. Signed CAIR, Parent Consent, Code of Conduct forms

Get a COMFORTABLE pair of black tennis/athletic shoes
  • You are going to be on your feet the entire day so comfortable shoes are a must. Mostly black shoes are required.
