Summary
Apple Patch supports individuals with intellectual and developmental disabilities, helping them lead fulfilling lives. The Community Living Support (CLS) Direct Support Professional provides one-on-one, individualized support for participants of all ages. Employees help people take charge of their own lives byteaching life skills that will enable them to be more independent within their home and community. Employees assist participants with developing friendships, volunteering, learning skills of money management and budgeting, remaining physically active and healthy, and promoting regular community engagement. A typical day could include going to the Y, the library, a volunteer job at a hospital gift shop, or simply helping someone organize their laundry. Creatively enabling people to meet their own goals in a variety of community locations is an opportunity to make a direct impact in the life of another. Organizational Duties and Responsibilities
- Supports the mission, vision, and philosophy of Apple Patch. Facilitates and supports the implementation of policies and procedures.

Essential Job Duties and Responsibilities:
- In conjunction with the team, develops and maintains a schedule of activities based upon the interests of the individual served. Provide services on a one-to-one basis.
- Supports the individual served to facilitate independence and promote integration into the community.
- Reports serious incidents, accidents, behavioral episodes and other matters requiring immediate reporting to proper person or authority.
- Keeps supervisor apprised of problems, potential problems and other issues to avert crises.
- Assists the individual served in any or all of the following areas:Routine household tasks and maintenance, Activities of daily living, Personal hygiene, Shopping and money management, Socialization, Relationship building, Leisure choices, Participation in community activities, Therapeutic goals, Non-medical care.
- Followsindividual service plans for each individual client.
- Maintainsdetailed documentation of participant progress and activities.
- Completes other documentation as required.
- Offers basic first aid and seizure monitoring, as needed.
- Regularly attends mandated trainings and staff meetings.
- Works collaboratively with other members of the client’s team.
- Performs other duties as assigned.

Education and Experience
High School Diploma or GED required. Bachelor’s degree in a human services field or in another field from an accredited university preferred. Previous experience working with individuals with developmental and intellectual disabilities preferred. Must have access to a vehicle to use for the provision of CLS services; must possess a current and valid driver’s license with an insurable driving record. Must have basic computer competency. Must be able to read, write, speak, and understand the English language.
