Who’s Eligible?

Associates budgeted for 20 hours or more per week are eligible for benefits after three months of employment.

New Hires eligible for benefits

Your benefits are effective after your have met a 90-day waiting period from your date of hire. However, you have the option of electing “First Day Benefits” for medical benefits only. If you elect this option, your medical benefits become effective on your date of hire. During this time, you pay the full premium for the first 90 days. After the 90 days, the regular subsidized contributions begin.

Status Changes/ Transfers eligible for benefits

If you have worked at Lourdes for at least 90 days your benefits become effective the first date of your status change. If you have not met the 90-day waiting period your benefits become effective after that waiting period has been met.

Choose Your Benefits Carefully

You will be able to review and update your benefit elections within the first 31 days of your date of hire or status change. If you do not enroll within 31 days of your date of hireor status change, your next chance to enroll will be during Open Enrollment in November 2012with a January 1, 2013 effective date.

Accessing Lawson Employee Self-Service (ESS)

You must log onto the Lawson Employee Self-Service (ESS)site and enter your user ID and password. Then, follow the prompts to complete your online enrollment within 31days of your date of hire or status change effective date. After you enroll, be sure to print a confirmation statement for your records.(Note: If you have not received your user ID and password within the first 31 days of your date of hire, you MUST contact the Lourdes Benefits Center – failure to do so will prevent you from being able to enroll in your benefits).

YOU MUST ADD YOUR DEPENDENT INFORMATION BEFORE ENROLLING FOR BENEFITS

To add your dependents (if applicable), follow these easy steps:

From the menu on the left under Employee Self-Serviceclick”Personal Information”then click on”Dependents”— this must be done before enrolling.

Click ”Add”to enter the dependent information on the main screen.

Click”Update”on the main screen to save your entry and continue enrolling.

LOURDES HEALTH SYSTEM (LHS) REQUIRES THAT ASSOCIATES PROVIDE PROOF OF THEIR RELATIONSHIP WITH THEIR DEPENDENTS WITHIN 31 DAYS OF ENROLLMENT

You are required to submitdependentdocumentation to the Lourdes Benefits Center within 31 days of enrolling your dependents or they will be terminated off of your benefits!!Refer to the attached chart for documentation requirements.

Fax copies of the required documents directly to the Lourdes Benefits Centerat 856-365-4088.

We recommend you call the Lourdes Benefits Center at 855-Ben-Yes1 (855-236-9371) to confirm receipt of your fax.

Dependents will be terminated if documents are not provided within 31 days of enrollment.

Enrolling For New Hire Benefits

Choosing Benefits

Log into Lawson Employee Self-Service (ESS) from any computer that has internet access (local library, friend’s home, etc.):

Once logged into the ESS system, select”New Hire”from the menu on the left hand side and click”Benefits,”then select ”New HireEnrollment.”

Click on”Enroll in 2012Benefits”on the left hand side. A welcome page will appear; scroll down and click”Continue.”

To enroll in benefits or to make a change, click”Continue.”

Make a selection for each benefit plan (e.g. change coverage, keep the same coverage, select a different plan) and click”Continue.”

Once you complete the entire enrollment process, click”Update”to process the changes for 2012.

Once you update, a pop-up box will appear to confirm your elections; click”OK.”

A screen will then appear asking if you want to print a reference page for your records; click”Yes”and print your reference page.

Your elections did NOT go through if your reference page states “You did not choose to keep thesebenefits. If you want these benefits, you must complete the election process again.”

Don’t have access to a computer? At Lourdes Health System (LHS), kiosks are located in the OLLMC & LMCBC cafeterias and in the lobby of the OLLMC Human Resources office – just click on “Lawson Portal”

Remember, you MUST enroll within 31 days of your date of hire or status change!

Dependent Documentation Requirements

You must fax the required documents to the Lourdes Benefits Center at 856-365-4088 within 31 days of enrolling your dependents.
