The following questionnaire is provided as a service to everyone who visits our website.
Buyers, please print this and bring it with you when you make an offer on real property.
Sellers, review these questions and resolve any concerns prior to listing property for sale.
The inspector is not liable for the accuracy of third party information. Contained within are the questions required by the Commonwealth of Massachusetts 266 CMR 6.03(4).
To the best of your knowledge as the Seller or Seller’s Representative:
  • Are there any flooding, drainage, or grading problems that have resulted in standing water on any portion of the property?
  • Is any portion of the property situated on or near a landfill?
  • Are there or have there ever had been fuel storage tanks above or below the ground on the property?
  • Has there ever been a leak in any fuel storage tanks?
  • Has motor fuel, motor oil, home heating fuel, lubricating oil or any other petroleum product, methane gas, or any hazardous waste or toxic substance spilled, leaked or otherwise been released on the property or from the property onto any other property?
  • Are there any hazards or hazardous material on the property, such as asbestos, dumps, pesticides, radon, and lead-based paint?
  • Are there any hazards or hazardous material in close proximity to the property, such as asbestos, dumps, pesticides, radon, and lead-based paint, fuel storage tanks above or below the ground?
  • Has the property been tested for the presence of any hazardous or toxic substance?
  • Is the property within an area currently of environmental concern? (superfund or wetland area).
  • Is there asbestos in the building?
  • Has the building ever been tested for lead paint or radon gas, If so, when?
  • Has the building been inspected by a home inspector. If so, when? Were any problems noted? Is a copy of the report available?
  • What is the age of the building(s)?
  • Is there any fire damage to the building(s)? If so, what areas were involved, what cleaners were used and when?
  • Is there, or has there ever been any, termite, insect, rodent or pest infestation, problems or damage?

  • Has the property been inspected for termite, insect, rodent or pest infestation, problems or damage?
  • Is there a pest or termite contract in place?If so, what chemicals were used?
  • Are there or have there ever had been any roof leaks or other roof problems?
  • When was the roof last repaired or replaced?
  • Is there a transferable warranty on the roof in effect now?
  • Does the basement have seepage that results in standing water?
  • Does the basement or crawl space become wet or damp? Has a dehumidifier ever been used in the basement?
  • Does the property have a sump pump or has one ever been used?Is there a French drain or perimeter drain?
  • Have there been any repairs or other attempts to control any water or dampness problems in the basementor crawl space?
  • Have you used a dehumidifier in the basement or ground floor areas of the building?
  • Are you aware of any past or present movement, shifting, deterioration, or other problems with the walls, slab, foundation, driveways, walkways, decks or patios?
  • Are there any problems with the heating or cooling system(s)? Date last serviced or repaired?
  • If your drinking water is from a well has the water quality and/or flow rate been tested?
  • If there is a septic or cesspool system, what is the age? Date last pumped? Frequency of pumping? Any maintenance agreements? Has a Title V inspection been completed and is the report available?

  • Is there any history of leaks, backups, or other problems relating to any of the plumbing, water, and sewage-related items?
  • Is there anunderground sprinkler system? Has it been properly winterized?
  • Are there any problems with the pool, hot tub, spa, sauna or their mechanical systems?
  • Is the seller aware of any structural, mechanical, electrical or other material defects that may exist on the property?
  • Has the seller or others made any additions, structural changes, or other alterations to the property?
    If yes, did you obtain all the necessary permits and approvals and was all work in compliance with building codes?
  • Are any of the items listed below included in the sale of the property? Refrigerator, Microwave Oven, Disposal, Washer, Dryer, Humidifier, TV Dish, Transmitters for garage door operator(s), Security Alarm System, Hot Tub, Pool Equipment, Solar Heating Systems, Wood Burning Systems, Intercom, Draperies, Light Fixtures.
