Applications are invited for this Specialty Grade post based at LeverndaleHospital. This is a challenging and rewarding post involving the care of patients with long-lasting and severe mental illness. Care is provided on a 12-bed rehabilitation ward with outreach into the community, to a 20 bedded slow-stream and a continuing care ward. There is an active discharge programme, and the team continues to be involved with those patients until they can be further referred back to the locality CMHTs. The Rehabilitation Service is being developed, and you will play a role in this.

The Speciality Grade would work as a core member of the multidisciplinary team and provide a comprehensive assessment and treatment service for the patients. AMP status is desirable and training towards AMP status would be supported. There is an active teaching programme at Leverndale, and continuing Professional Development, Audit and Research are encouraged and supported. You should have full medical registration and a minimum of 3 years’ experience of working in psychiatry is desirable.

This is an 8 session post and all duties will be undertaken during working hours. There are no on-call duties.

Rehabilitation Psychiatry

  1. Current PSYCHIATRIC SERVICES- SOUTH GLASGOW

Mental Health Services in Glasgow are delivered in conjunction with the Glasgow CHP and other outlying CHCPs for the whole of Greater Glasgow. The Lead Associate Medical Director for Health and Learning Disability is Dr Michael Smith, who is based at William Street Clinic. The Management Team for South Glasgow is based at LeverndaleHospital; the Head of Mental Health is Mr Calum MacLeod and the Clinical Director is Dr Gwen Jones-Edwards.

The mental health services in South Glasgow provide care for a total population of 320,000. Community adult services are based in seven general adult Resource Centres. The acute inpatient beds are based across three wardsatLeverndaleHospital. The regional Mother and Baby MHU is also housed on site in a new high quality unit. There are excellent clinical and non-clinical support services available including the full range of psychiatric sub-specialities. Community services are well established and based around generic community mental health teams enhanced by the Crisis teams, a First Episode Psychosis team (Esteem), a Perinatal team, an Eating Disorder team, Addictions teams and Primary Mental Health Care teams.

LeverndaleHospital has 216 inpatient beds. This includes 72 acute general adult beds, 12 intensive psychiatric care unit beds, 14 rehabilitation unit beds, 58 acute old age psychiatry (18 organic and 40 functional), 48 forensic beds and 43 adult medium stay beds. There are also 150 long stay elderly dementia beds split between Darnley (60) and Rutherglen (90), and 63 adult long stay partnership beds.

There are 7 General Adult Resource Centres covering Gorbals/Govanhill, Castlemilk, Rutherglen/Cambuslang, Govan/Ibrox, Pollok/Pollokshields, and Barrhead/Eastwood. Child and Adolescent Services are based at the TwomaxBuilding in the Gorbals, but the inpatient beds are provided at the Skye Unit at StobhillHospital. Learning Difficulty assessment beds and 20 continuing care beds at GartnavelRoyalHospital. There is also a Community Learning Disability Team.

3.OVERVIEW

Clinical Work

The Rehabilitation Unit is based on the Leverndale campus and it comprises of an intensive rehabilitation unit, an intermediate intensive ward and a slow-stream unit.

The intensive rehabilitation unit is a 14-bedded unit, where individuals tend to have a length of stay between 6 to 18 months.

On the intensive unit, the postholder will be expected to undertake the psychiatric and clinical care of the rehabilitation inpatients, under the supervision of the Consultant Psychiatrist, Dr Ciara Kelly, and in keeping with the Multidisciplinary Rehabilitation Assessment and Care Planning.

The Rehabilitation Unit is a non-acute setting, although there are infrequent direct admissions of well known community patients organised by the Outreach Service.

The Outreach Service is the Community Rehabilitation Service, which is also based on the Leverndale campus. Their role is to form an intensive Outreach/Rehabilitation Model to support patients with complex needs in the community setting.

Referrals are made to the Rehabilitation Service and patients are accepted as either inpatients or community referrals. The nature of individuals referred to the Rehabilitation Service are that they are often difficult to engage, many are detained under Mental Health Act or Adults with Incapacity legislation, and they are high users of Psychiatric Services. The Rehabilitation Service has a comprehensive, rehabilitative and multi-disciplinary approach to promote improvement and re-engagement with community living.

Patients are referred from the acute adult inpatient services in Leverndale and the Southern General, as well as from Learning Disability, Forensic and occasionally from Adolescent Services. 60% of the referrals arise from the inpatient setting and 40% are community patients with rehabilitation needs.

As part of the Rehabilitation Service, the postholder will be involved in the engagement and assessment of individuals, in a multi-disciplinary team approach and under the supervision of the Consultant Psychiatrist. The Specialty Grade post will be regarded as a core member of the clinical team and it is expected that he or she would train to become an approved medical practitioner under the Mental Health Act when eligible.

The Rehabilitation & Medium Stay service is a key component of mental health services in South Glasgow. It has a current staffing level of 1.3 Consultant Psychiatrists, 0.8 Speciality Grade Doctor, 1 wte Consultant Psychologist, 1 Senior Occupational Therapist and 1.5 Occupational Therapy technical support. There is access to wider input from Physiotherapy, Dietetics, Speech & Language Therapy and Clinical Pharmacy based within Leverndale.

Slow Stream Ward

Ward 2 had 23 beds (of which only 15-20 are being currently utilised) and functions as a slow stream unit for individuals who have high levels of disability and complex co-morbid conditions but relatively lower levels of active symptomatology. The ward offers a high level of practical support with emphasis on promoting social and interpersonal functioning in the longer term.

Intermediate Rehabilitation Ward

Balloch Ward has 20 beds and functions as an intermediate stream rehabilitation unit. The focus is on a client group who have both active symptomatology and impaired functioning, associated with more challenging behaviour and/or behavioural disturbance. The focus is on engagement, thorough ongoing assessment, maximising benefits from medication, reducing challenging behaviours, and re-engaging with families and communities. Family work is supported via Behavioural Family Therapy Model. The goal is a move onto the intensive rehabilitation unit and ultimately supported community living. On the slow-stream and intermediate wards you will work under the supervision of Dr Andrea Perry. The ward is supported with MDT working with both Consultant Clinical Psychologist and Senior Occupational Therapist input.

It is essential that a successful applicant would be an approved medical practitioner in terms of Section 22 of the Mental Health (Care and Treatment) (Scotland) Act 2003 or be eligible for AMP training. There are opportunities to develop psychological competencies.

4.TEACHING

LeverndaleHospital is a teaching hospital and, as such, it aims to provide high quality teaching for all health care personnel. Medical students from GlasgowUniversity are attached to the consultant psychiatrists on a 5 week clinical clerkship basis. The students receive teaching in both the hospital and community settings to enable as rounded an experience in psychiatry as possible. The speciality grade psychiatrist will be expected to participate, under supervision, in informal and formal teaching of medical students, as well as teaching students of nursing, social work, and occupational and other therapists. The speciality grade psychiatrist would be encouraged to participate in audit and research and ongoing projects with supervision.

5.OCCASIONAL EMERGENCIES

The speciality grade doctor will also perform duties in occasional emergencies between the hours of 9am and 5pm (including detention of inpatients on STDC when the patient’s own Consultant was not on site) and, in unforeseen circumstances, at the request of the Clinical Director, in consultation where practicable with his/her colleagues both senior and junior.

6.CONTINUING EDUCATION

The postholder will be encouraged to utilise his/her full entitlement to study leave. Attendance at fortnightly case conference/journal clubs would be expected and individuals could negotiate attendance at the external MRCPsych course, if they were planning to sit exams, or if this was required for their Continuing Professional Development. The facilities of the medical libraries at LeverndaleHospital and the Southern General Hospitals, as well as the main library at GartnavelRoyalHospital, will be available. There is also an online library, which includes a literature search facility, to which all staff may have access. Advice on continuing postgraduate education and careers counselling is available from both the college tutors and other senior staff.

SUPERANNUATION / New entrants to NHS Greater Glasgow and Clyde who are aged sixteen but under seventy five will be enrolled automatically into membership of the NHS Pension Scheme. Should you choose to "opt out" arrangements can be made to do this via:
