1.  Motivation and effort (40 pts)

a)  Did your leader have a presence at planned CFD events?

b)  Give examples of different support during the campaign.

c)  Describe the value your leader brought to your campaign.

Award Criteria

In order to establish full transparency, the Combined Fund Drive uses the following criteria to judge each award nomination:

1.  Nominations are emailed to by January 8, 2016.

2.  Each nomination is made anonymous.

  1. Names and contact emails are eliminated.
  2. Nomination forms are judged solely based on content.

3.  Each CFD staff member rates nomination forms separately.

  1. Outreach Deputy Director
  2. CFD Program Manager
  3. Resource Coordinator
  4. Program Specialist

4.  Aggregate scores from each nomination for are added to establish final scores.

5.  Outreach Director approves the winner for each category.
