Context:

The post holder will be working within the Supported Living Team, part of the wider Commissioning, Contracting and Market Management Team and will be responsible for assessments and support planning for vulnerable adults across all service user groups.

Purpose of Job:

  1. To carry out needs led assessments of adults within the guidelines of Care Act 2014
  1. To care manage straight forward and complex cases including Safeguarding Adults and Deprivation of Liberty. Co-ordinating and reviewing multi-service care packages.
  1. To support the unit in meeting individual / Unit performance targets and actively contributes to its continuous improvement.

Principal Accountabilities

  1. To carry out needs led assessments of service users and their carers in accordance with the council’s self-directed assessment.
  1. To formulate appropriate care and support plans and negotiate care packages with service users and service providers. This will involve formulating multi-service care packages, and reviewing on a regular basis.
  1. To carry out all duties in accordance with the council’s policy and procedures.

Section A: Main Duties & Responsibilities for all posts:

  1. To carry out needs led assessments of service users and their carers and to manage cases on a time limited or long term basis and meet performance targets.
  1. To work with service users, carers and other professionals to minimise and manage risk whilst enabling the service user to maintain their chosen lifestyle as far as possible.
  1. To complete financial assessments of service users, and support them to manage their financial affairs.
  1. To apply fair access criteria to determine eligibility for treatment.
  1. To undertake Mental Capacity Assessments in accordance with the Mental Capacity Act 2005.
  1. To identify appropriate services to meet appropriate need, and negotiate contracts with providers.
  1. To provide care and support plans to service users in receipt of services from the department.
  1. To complete Court of Protection applications in conjunction with Brent’s legal services.
  1. To regularly monitor and carry out within guidelines of the Care Act timely annual reviews of services provided to service users, liaising with carers and stakeholder professionals. This will include reviewing multi-service care packages.
  1. To update care and support plans following reviews, and negotiate care packages with service providers.
  1. To be familiar with Brent’s policies and procedures and appropriate legislation procedures and resources.
  1. To maintain appropriate and up to date service user’s records and to provide statistical information and reports as and when required. To co-operate in research projects where appropriate.
  1. To contribute to the development, planning and implementation of policy and practice relating to the care management of service users.
  1. To facilitate and foster effective relationship between local authority, NHS, private and sector partners and service providers.

Standard Requirements of all post:

  1. To maintain Social Worker Registration through the Health Care Professional Council (HCPC), and observe the professional code of conduct at all times.
  1. To engage in regular vetting with the Criminal Records Bureau.
  1. To have responsibility for a portfolio of cases for a defined area of work as determined by the line manager.
  1. To respond to Emergency situations when required and participate in an annual leave cover system to oversee colleague’s allocated cases as determined by the line manager.
  1. To promote and ensure maximum service user/carer’s involvement and participation at all times. Engaging advocacy services when required.
  1. To ensure that care purchases are set up to reflect customers need and are best value for money.
  1. To maintain such reports and records as may be required, including car mileage expenses, hours worked, daily diary etc. in accordance with Brent’s policy and procedures.
  1. To participate in professional supervision with the Line Manager.
  1. To undertake other duties, commensurable with the grade of the post as may be allocated by the post holder’s line manager.

Section B: Context for Supported Living and other accommodation Team Duties:

The designated Social Worker will:

  1. Assess and review vulnerable adults across all service user groups living in supported living accommodation.
  1. Develop person centred care and support plans in conjunction with service users, carers and other professionals promoting the personalisation agenda through use of personal budgets and direct payments.
  1. Undertake mental capacity assessments around change of accommodation, tenancies, finances and community based deprivation of liberty and make Court of Protection applications.
  1. Develop and promote the role of social work within the wider Team.
  1. To attend and contribute effectively to team meetings, case reviews and other appropriate meetings.
