FIELD TRIP ACKNOWLEDGEMENT / RELEASE / WAIVER / PERMISSION FORM

This is to certify that ______(“Participant”) has full permission to, and/or has voluntarily agreed to participate in a group organized by Savannah Science Seminar or independently in connection with a field trip to Kraton Polymers LLC (“Kraton”) to participate in the activities outlined in the attached Exhibit A (the “Activities”) at a Kraton’s facility (the “Facility”). As Participant / Parent or Guardian of Participant:

I agree to follow all rules and instructions established and provided by Kraton regarding conduct, health, safety and fire protection at the Facility at all times.

I understand that participation in the Activities and / presence at the Facility could involve risk of physical injury, illness, death or property loss, and despite safety precautions, Kraton cannot guarantee safety thereof, as all risks cannot be prevented. I hereby expressly assume all risks, known or unknown, which could occur through participating in the Activities. Kraton does not provide health and accident insurance for Participants in the Activities, and I understand that any medical expenses, property loss, or other personal expenditures that result during or from participating in the Activities, are to be borne by the Participant, or by their parent or guardian (if Participant is a minor). I also hereby consent, release from liability, and give authorization to Kraton to secure any emergency medical treatment necessary in event I am unable to do so, and I agree to be responsible for the costs thereof.

I further acknowledge that if I drive my own vehicle, or am a passenger in another’s private vehicle in connection with the field trip to the Facility, that Kraton’s auto insurance does not cover such a private vehicle. I also understand that Kraton cannot be responsible for assuring the safety and reliability of such private transportation or driver, nor for any activities and travel that I / my child might choose to participate in before, during or after the visit to Kraton, and I therefore accept the risks and responsibilities associated with such private vehicle travel and activities.

In consideration of the opportunity afforded, with full knowledge and acceptance of the risks associated with this trip and the Activities, and with full understanding of the issues/conditions and risks associated with the Activities and / or the Facility, I for myself, my heirs, executors and administrators, hereby release, indemnify and forever hold harmless Kraton, its directors, officers, employees, agents, affiliates, subsidiaries and assigns from all form and manner of risks inherent in, and from all claims, suits and demands of any nature arising from participation in said trip to the Facility and / or Activities.

This Release form is intended to be as broad and inclusive as permitted by law, and if any portion of this document is found to be invalid, the rest of this document will remain in effect. This Release shall be binding on my heirs, executors, administrators and assigns.

I acknowledge that I have read this Release in its entirety or that it has been read to me if I have been unable to read it.

Possible risks associated with the above activities may include but are not limited to exposure to room temperature resins, e.g., rosin acids, terpene resins, rosin esters, etc., which may be skin irritants. There can be flammable solvents, materials found on the OSHA Substance Specific Standards list, materials deemed hazardous substances (e.g., carcinogens, toxic, teratogens, etc., as commonly found in chemical laboratories). However, these materials are kept in secured Flammable Storage Cabinets and will not be utilized in the Activities.

Kraton will provide and require Participants to put on personal protective equipment (PPE) such as safety glasses prior to entering the Laboratory.

For the FTIR (Fourier Transform Infrared Spectroscopy) activity, Participants will be required to wear lab coats and nitrile gloves for handling resins. For other activities, Participants will work with computer software at various instrument stations in the Laboratory.
