The First Tee of Benton Harbor 2017 Scholarship Application
Thank you for your interest in The First Tee of Benton Harbor 2017 Scholarship. Each year, the chapter selects one graduating participant to receive $5000 and another to receive $1,500, to be used toward continuing education after high school.
The scholarship committee will review completed applications, and the winners will be announced during the annual The First Tee of Benton Harbor Scholarship and Volunteer of the Year Award Dinner on May 4th, 2017.
Application Guidelines
Your completed application must be submitted to The First Tee of Benton Harbor,201 Graham Ave., Benton Harbor, MI 49022, no later than April 1, 2017. You may email it to Ebon Sanders at , drop it off at the Academy or mail it to the above address.

For mutual consideration, the receipt and sufficiency of which is hereby acknowledged, I hereby assign and grant to The First Tee of Benton Harbor, a chapter of The First Tee, a division of World Golf Foundation, Inc., the right and permission to copyright and/or publish and republish, and to sublicense to its sponsors and licensees, photographic pictures/video of the below named minor (“the Model”) in which the Model may be included in whole or in part, in color or black and white made through any media by, or on behalf of, The First Tee of Benton Harbor, including the use of any printed matter in conjunction with such photographs for advertising, trade or any other lawful purpose. I warrant that I am the parent or legal guardian of the minor named below and have the authority to execute this release on his or her behalf.
I hereby waive any right to inspect and/or approve the finished photograph or advertising copy or printed/digital matter that may be used in conjunction with or contain such photographs, or to the eventual use that it may be applied.
I hereby release and discharge to The First Tee of Benton Harbor from and against all liability as a result of any distortion, blurring, alteration or optical or computer illusion that may occur in the taking of the picture or processing or reproduction of the finished product.

The First Tee of Benton Harbor Social Media Code of Conduct
The First Tee of Benton Harbor Code of Conduct requires participants to demonstrate the Nine Core Values in their everyday lives and, in particular, to show respect for oneself, others and their surroundings. These caring behaviors are expected not only in face-to-face situations but also in written correspondence and electronic social media interactions. Before, during or after participating in an event sponsored by the The First Tee of Benton Harbor, you may want to communicate with other participants from the event. With that in mind, The First Tee of Benton Harbor has a social media code of conduct of which we would like to make you aware. For clarification, we consider social media to include all personal websites and all forms of online community activities such as online blogs, social networks, message boards, conversation pages, chat rooms, emails and cell phone texting.
The First Tee of Benton Harbor Social Media Code of Conduct states that while using any social media platform to communicate with other participants or with anyone about The First Tee, you must abide by the Code of Conduct and The First Tee Nine Core Values. If you are unaware of what the Code of Conduct states, you can review your Participant Yardage Book or get it from The First Tee of Benton Harbor Academy. Also, The First Tee’s copyrighted words, logos and trademarks may not be used in any manner on social media pages without home office approval. For example, you cannot create a social media group that appears to be sponsored by The First Tee.
It is very important to respect and follow the rules set forth in The First Tee Code of Benton Harbor Conduct and Nine Core Values when using any social media platform. If you fail to do so, there will be consequences, which may include cancellation of benefits and being prohibited from participating in future participant events hosted by The First Tee home office. The First Tee of Benton Harbor will also involve parents and chapter leadership in these discussions as needed. Thank you for your attention to these guidelines.

Essay Questions
Please answer both questions completely. One of the goals of The First Tee of Benton Harbor is to positively impact young people’s lives. Your answer to both of the following questions will help the Scholarship Committee understand and evaluate the effect our mission has had on you. Remember, you are being judged by a committee. Therefore, both essays should provide insight into who you are as a person. Your answers should be typed, double-spaced and contain between 200 and 300 words for each question.
Question 1: Describe in your own words the value that participating in The First Tee of Benton Harbor’sprograms has had in your life. Tell a story about yourself using one of the Life Skills or Core Values you have learned.
