Career Impact Award Nomination Instructions
The Career Impact Award is awarded annually to a person with 15 or more years as a Cooperative Extension Family Life or Human Development State Specialist.
Nomination letters should respond directly to the criteria below. Self-nominations are welcome. The nomination letter should not exceed three pages. In addition, provide an abbreviated vita for the nominee. The abbreviated vita should not exceed five pages. The nomination letter and the abbreviated vita should both be formatted with 1-inch margins and 12-point font.

B. Describe the nominee's significant contributions regarding two or three impactful Family Life and/or Human Development Extension programs (e.g., face-to-face courses, printed resources, train-the-trainer curricula, social media, on-line programs, etc.). For each, describe the nominee’s role (e.g., development, implementation, evaluation, etc.) and outputs (e.g., participants reached, copies sold, adoption/usage by other states, partnerships established, web analytics, etc.).
C. For each of the programs detailed above, describe significant outcomes and impacts (e.g., skills learned, policy enacted, conditions improved, etc.).
D. Describe the nominee's reputation, leadership, and influence both inside and outside their home state (e.g., regionally, nationally, on-line, within Communities of Practice, etc.).
E. Describe any unique circumstances, special considerations, or distinctive factors regarding why the nominee should receive the Career Impact Award.
