SELECTIONS AND PAYMENT –FINAL DUE DATE – JUNE 10, 2008

INSTRUCTIONS: COPY & READ THIS MATERIAL BEFORE COMPLETING THE FORM
1. Complete the identification information for the person who is making the payment.
2. For each person covered by the payment, list their full name and age (if below age 21).
3. For each person, put the corresponding “Letter” (A, B, C etc.) under the appropriate age
category at the top of the form (see example)
4. For each activity item, determine the fee for each person based on their age category.
Attendees age 16 to 20 pay half the adult fee.
Those under 16 are free.
Place a zero ( 0 ) if the person plans to participate but there is no fee to be entered.
5. Add the fees going across and put the result in the Total Fee column.
When all items for all persons are completed, then
6. Sub-total the amount for each person (columns with a letter at the top)
7. Add across the sub-totals for each person and put the result in the Total Fee column
8. Sub-total the amounts in the Total Fee column ( far right hand side)
The results of #7 and #8 should be the same.
9. Enter each $25 Deposit (if one was made) and place the sum in the Total Fee column
10. Subtract Total Deposit from the Total Fee sub-total to obtain the Payment Due.
Below is an example of a completed form for my own family group of eight attendees. This should help you complete your form. If you do your own taxes, then this will be easy.

**The Loch Etive Cruise is a commercial operation. The fee will be paid when you board the boat. The fee schedule is in PROGRAM DETAILS – Loch Etive Cruise.

If the person is only attending on Saturday and Sunday, the registration fee is $30.
Once your payment is received, there will be a form at the web site where you can make your 1st and 2nd choices for activity dates.
You can make changes until June 15th. After that date the Priority Number will no longer be valid but you can make changes at the Gathering, if space is available.

Please send Payment by Check or Online by PayPal®.PERSONAL CHECK (Preferred method for applicants with US Dollar Checking Accounts):
Make the check out to: Martin MacIntyre. On the check notation line write 2008 Gathering.
Attach a copy of the Payment Form with the names of those covered by the Payment.

Print these instructions, read them. Then go to the Menu page and click the Donate button at the top.
NOTE: Sorry, although the button says “Donate” it really is just your payment for the items you selected and isn’t tax deductible. This is the only way the PayPal system allows a single payment without itemizing. That information is on the Selections & Payment Form that you have just completed.
1. On the PayPal® page at the top right it says MacIntyre Publishing, which is the name for my (Martin MacIntyre’s) PayPal® business account.
2. Enter the Payment Due amount in the TOTAL window.
3. Click the “Update Totals” button.
IGNORE the LOG IN window.
4. Go to the left side near the bottom where it says:: Don’t have a PayPal account?
This will apply to almost everyone you. Click “continue”.
5. PayPal® will ask for your bank credit card information, billing address, e-mail address and telephone number. Once this is completed, click “Review Order and Continue” and your payment will be processed.
6. You must send me a copy of your completed SELECTIONS & PAYMENT FORM including the names of each person covered by the payment. This is the only way for me to know who is covered by the payment and which items have been selected.
7. Return to the Menu Page. Click the “Donate” button and enter the Payment Due.
8. PayPal® will send both of us an e-mail summary and confirmation number.
