RE-REGISTRATION FORM

(One per child, please)

PLEASE RETURN NO LATER THAN FEBRUARY 18, 2016

1. All re-registration forms must be accompanied by a $150.00 registration fee. This is a

$150.00 fee per family and is non-refundable. Failure to send in payment may result in

losing the spot to another student should the class become full.

2. Transportation request forms are enclosed (if available) and are to be filled out if applicable.

Transportation requests must be received by the districts by April 1st to ensure

transportation for the 2016-2017 school year. Don’t be late! Transportation forms sent

in to the HCS office will be sent to the districts on March 18th.

3. All present families will be accepted first (if registered by February 18). Registration will

then be opened up to outside families. Please be aware that there is a limit of 20 students in

Kindergarten and 1st grade, a limit of 22 students in 2nd-5th grade, and a limit of 25 students

per middle/high class. To hold a space for your child, forms must be returned promptly and

accompanied by fees. Students registering for classes already full will be placed on a

waiting list.

4. If you do not plan to send your child to Harmony Christian School next year, pleasesend in

this form indicating this and stating your reasons.

5. In order to ensure staffing for our extended daycare program it is important that you

indicate your interest by checking the appropriate space at the bottom of this form.

6. If you have any questions, please feel free to contact us at 692-5353. Anyone unsure as to

whether or not their child(ren) will be attending should consider sending in transportation

requests by April 1st so transportation will be reserved in case it’s needed.
