NORTHCAROLINA-CALDWELLCOUNTY
AGREEMENT, made and entered into this______day of______by and between the Caldwell County Board of Education, hereinafter referred to as the Local Education Agency
(LEA), and ______hereinafter
referred to as the Contracting Agency.
WITNESSETH:
I. The Local Education Agency hereby agrees to:
a. provide appropriate financial consideration for the cost of performance of this agreement at the
rate of $______not to exceed $______. The LEA
hereby agrees to make payment on an actual cost reimbursement basis to the Contracting Agency upon submission of the proper documentation of cost incurred.
II. The Contracting Agency hereby agrees to:
a. to furnish transportation for ______(child/children) to the
______(School).
b. Furnish signed invoices indicating the number of trips or bus fares covered in the pay period,
the number of pupils transported, the dates of the trips, and the miles covered under this
Agreement.
c. submit miles traveled as those miles which indicate the distance the child travels.
d. file the report no later than the 5th day of each calendar month, with the final report being due
no later than June 15, 2018.
e. to hold a valid driver’s license and carry proper liability insurance in the event of an accident.
(The carrier is liable in the event of an accident resulting in property damage, personal injury
Or both.)
III. The Local Education Agency and Contracting Agency hereby mutually agree that amendments to or
dissolution of this Agreement may be made during the term of the Agreement by giving written notice
30 days prior to the termination of the contract. It is further agreed that upon dissolution, the Local
Education Agency’s financial liability shall be on the actual cost incurred during the term of this
Agreement by the Contracting Agency at the time of termination.
IV. Terms of Agreement:
This Agreement is entered into the aforementioned date and shall remain in force and be mutually binding
upon the parties hereto from the period of ______,
through______unless sooner amended or dissolved by
either party hereto as described in (Section III) above.

which is retained by each of the parties, the day and year first above written.
