- For PC 1 – 8 use this form in your faculty/departmental recruitment process, guided by the PASS recruitment policy (payclasses 1 - 8).
- For PC 9 – 13:
-Refer to the PASS recruitment policy (payclasses 9 -13)
-Complete this selection committee form and return to the HR Practitioner for your area.
-In addition, complete form HR202(Request to commence recruitment process) and return to theHR Practitioner for your area.

When do I complete this form?
This form is used when a department appoints a selection committee to recruit and fill a vacant PASS post.
It also details the composition of the committee for each.
- ALL members of the committee should be advised by the HOD/Dean that they have been nominated to serve.
- The committee should be representative in terms of race and gender.
Where do I send this form?
For PC 1 – 8, use this form in your faculty/departmental recruitment process, guided by the PASS recruitment policy (payclasses 1 - 8).
For PC 9 – 13, departments should send this form to the HR Practitioner for your area.
When do I submit this form?
As soon as possible.
What other forms do I need to complete?
For PC 9 – 13, form HR202 (Request to commence recruitment process) must also be completed.
