Title of Procedure: Utilization of the Position and Personnel Action Form

Process Step / Action To Be Performed By: / Actions To Take
Termination of Employment
Required for all employees without a specified contract end date. / Supervisor / 1.  Check “Termination” in Section 1.
2.  Complete Section 3 with all pertinent employee information:
·  Banner ID
·  Employee Name
·  Position Number
3.  Complete Section 8 with all pertinent separation information:
·  Termination Effective Date
·  Last Day Worked
·  Termination Reason
4.  Attach supporting documentation, i.e. resignation letter.
5.  Obtain appropriate signature in Section 9:
·  Department Supervisor or Department Chair
·  Dean (for EPA teaching positions)
·  Provost and Vice Chancellor for Academic Affairs (for EPA teaching positions)
·  EPA Contract Administrator (for EPA teaching positions)
6.  Submit to HR Services Manager.
HR Services Manager / 7.  Notate date received in Section 10. Review form for accuracy and completeness. Obtain supporting documentation as appropriate, if not attached.
8.  Obtain employee’s signature.
9.  Give completed form to Administrative Support Specialist.
Administrative Support Specialist / 10.  Enter data into Banner. Notate date entered in Section 10.
11.  Follow filing SOP.
