Dr. _____ has made a major contribution to the Faculty’s educational programs, particularly at the undergraduate level. He/She has played a primary role in the extensive changes that have taken place in the medical curriculum over the past five years. As the program director for clinical skills, he/he has shown excellent leadership in the organization and implementation of the communication skills part of the program, one of the first of its kind in Canada. He/She has published work in the area of medical education and presented at national and international meetings. In 19__, he/she was the recipient of a Faculty of Medicine Teaching Award. Within the Department of ______’s graduate program, Dr. _____ has functioned as thesis supervisor and member of thesis examination questions. He/She is well regarded as a teacher and supervisor within the department’s residency training program.

Born in ______and educated at Queen’s University, Dr. _____ received his/her PhD in 19__. After a postdoctoral fellowship in the laboratory of the late Professor _____, he/she was appointed Assistant Professor in the Department of ______. He/She received tenure in 19__ and promotion to Associate Professor in 19__. In 19__, Dr. _____ became director of ______in the Hotel Dieu Hospital and in 19__, was appointed Associate Professor in his/her cross appointed department of ______. From 19__ to 19__, he/she served as Associate Dean of Graduate Studies and Research. He/She has held two visiting professorships, one in New Zealand and the other in England.
Dr. _____’s prowess as an educator has been recognized by receipt of the 19 __ and 19__ Aesculapian Society Awards for Excellence in Teaching and a 19__ Faculty of Medicine Education Award. He/She is acknowledged nationally and internationally for his/her scholarship in educational technology, and he/she is the co-developer of a comprehensive CD-ROM teaching program.

Dr. _____ obtained his/her medical from Queen’s University in 19__. Following five years of postgraduate training at Queen’s, in Toronto and San Diego, California, Dr. _____ took up an appointment as Assistant Professor in the Department of ______. He/She was promoted to Associate Professor in 19__ and was granted tenure in 19__.
Dr. _____ is a respected clinician, researcher and educator. With his/her caring, inquisitive, and compassionate approach to patients and their families, Dr. _____ is a role model for students and trainees. His/Her research interests have focused on the therapy, course and complications of rheumatoid arthritis, with particular interest in the hand function in patients afflicted with this disorder. He/She is currently involved in the analysis of a 3-D optoelectronic motion system for the hand and fingers. Dr. _____ played a key role in the introduction of the Faculty of Medicine curriculum. His/Her contributions to the development and implementation of this major innovation were recognized by the awarding of the Faculty of Medicine Education Award. It is significant also that the graduating class chose to recognize these contributions by the selection of Dr. _____ for the W. Ford Connell Award for contributions to teaching.
