Transition Planning Checklist
This transition checker is in support of the Transfer Protocol between Children and Adult Services- to follow.
There is a commitment in the protocol to a formal transfer meeting held between the young person’s social worker in Children’s Social Care and their newly allocated worker in Adult Social Care.
The young people in the priority groups are those who:
- AreLooked After(outside of their family for more than 75 nights a year);
- Have High care support needs; or
- Where there are particular issues were a young person needs an exceptional level of support to manage risk in their day to day life.
The meetings for these young people will be chaired by the Transitional Worker and will be organised by the Children’s Social Care social worker. For all of the other young people that we work with the two social workers involved will meet together.
his meeting should happen no later than three months before the young person’s 18th birthday.
A young person will move to Adult services with these documents:
- The right assessment for them so depending on their circumstances this will be an initial core or for young people who are looked after aPathway Plan;
- ACare Planor a Child Protection Plan if they are the subject of one;
- A genogram;
- An up to dateChronology;
- A list of key contact people involved in the young person’s life particularly colleagues in health services;
- Where there is an identified risk for a young person that an up to date risk assessment is available;
- Where there are issues of consent any assessments of capacity that have been made post 16 are highlighted and discussed;
- A transitional plan;
- Were the young person is moving in to adult life with an individual budget their support plan should be available.
Meetings will concentrate on what is in the transitional plan. This should address all issues in the transitional planning discussions that have taken place over the last couple of years. It should also highlight what needs to be done moving forward into adult life. It should use the five outcomes to identify what is to be done.
The following is a checklist of issues that should be considered. Has the young person:
- Got a person centred or support plan;
- Got concessionary travel passes (bus and Metro);
- Has the question of independent travel been discussed and a way forward agreed;
- Have they had a benefits check since they were 16;
- Are the issues around transitions for health care needs been addressed?
- Funding of support services discuss what support is currently been provided through their child care plan and identify the weekly and annual cost of this support.
