Purpose of Job / To support clients in adult and YP services to engage with and access community treatment services through assertive outreach and open access services. To deliver a range of interventions including 1:1 and group work, psychosocial interventions as well as offering a range of harm reduction/preventative interventions such as needle exchange, educational group work and diversionary activities
Key Activities:
- Provide frontline interventions to clients to ensure delivery of a quality service inline with DANOS competencies
- To carry out triage and comprehensive assessments on clients referred to the service
- To deliver a range of individual treatment interventions to the clients utilising a person centred approach
- Provide out of hours service
- To work flexibly and in a holistic way to promote harm reduction
- To devise, implement and review individual recovery care plans on clients accessing the service
- To appropriately include parents/carers/concerned others in the planning and delivery of support
- To support a caseload of clients, delivering basic counselling skills, motivational interviewing & ITEP mapping
- To offer a flexible service across CGL Wales sites working closely in partnership with Health, Education, Social Care, Criminal Justice and generic family services.
- To accurately maintain a client management system and computer database
- Ensure information is shared in line with CGL’s information sharing guidance
- Provide specialist information and assistance regarding housing, finance, education, employment and training.
- Provide effective links with mental health, voluntary and statutory sector providers and agencies
- Do you own administration and contribute to the administration of the team
- To run or support group work sessions on particular topics as determined by service users need

Person Specification
Essential criteria:
Experience of working with substance misuse/emotional wellbeingissues and a clear understanding of the need for and ability to deliver quality services.
Maintaining professional boundaries and effective working within treatment services.
Knowledge of the issues facing service users including social care, welfare benefits, health and emergency accommodation
Ability to liaise effectively with other agencies
Ability to assess and care plan service users and formulate written reports as necessary.
Demonstrate skills to undertake administrative duties and manage data collection systems.
Experience of contributing to project development
A valid UK driving licence and access to own vehicle or demonstrate how you will meet the travelling requirements of the post.
And the ability to:
Communicate confidently and effectively, verbally and in writing.
Respond flexibly to the demands of the post including weekend and evening cover.
Work as a member of a team.
Show a capacity to work alone and the ability to keep calm under pressure.
