Our 2015 Personal Health Assessment (PHA) is a valuable opportunityto benchmark your health starting now. Part of the ongoing Bon Secours Health System’s Healthy Me, Healthy You Employee Wellness initiative, the PHA can help you know your numbers andPower Upto improve your health.

By completing both parts of the PHA, you will qualify to receive $300 in a Health Reimbursement Account (HRA), if you have elected a Bon Secours Medical Plan starting September 1, 2014.

Your PHA results not only empower you to make healthy changes, they also empower Bon Secours to develop targeted programs in response to what is learned from aggregate results.

  1. What is a Personal Health Assessment?

As the name implies, the PHA is a tool that enables individuals to evaluate their own health. Our PHA includes two parts:

  • Onsite Biometric Health Screening – Key biometrics like blood pressure, weight, cholesterol, and blood glucose levels are recorded for your ongoing tracking. There will be screenings available onsite at various Bon Secours locations between May and July, 2015. You can also have your physician complete and submit the PHA Biometric Screening Verification Form 2015, available to you online.
  • Online Wellness Profile –An interactive tool that will guide you through a series of questions about a broad range of health-related practices and indicators.

The PHA is voluntaryand family members are not required to participate, but if you are a benefit-eligible employee, your spouse or legally domiciled adult, is encouraged to attend. Please note that spouses need to complete the online wellness profile before attending an in-person screening.

  1. What did we learn from last year’s PHA?

Across Bon Secours, the top controllable risk factors are weight and BMI, sedentary lifestyle, hypertension, high cholesterol, and high stress levels. We are focusing on ways to address these issues by providing targeted, responsive programs to support our journeys to better health.

4. How do I access the Health Screening and Online Wellness Profile?

OnsiteBiometric Health Screening – The health screenings will be offered on a number of dates throughout Bon Secours Virginia in May and June and in all other health systems in June and July. Screening dates, times and locations can be found at under the “Calendar” tab and look for “PHA Screening” events.

Online Wellness Profile– The online wellness profile will be available at on April 1, 2015. If this is your first time logging-in, your user ID is your 5-digit employee number (Lawson #) and your password is your birthdate (mm/dd/yyyy) with no spaces or dashes. You will be prompted to change your password upon initial log-in.Please write down your newly created password as this will be needed each time you log-in to the wellness website. If you have previously logged into the website, your password is what you have already created.

5. Do I have to do the PHA?

No. The PHA is entirely voluntary. Bycompleting, however, it will help you take an important step on the road to better health and make you eligible for future wellness programs. Your participationwill help BSHSI develop Employee Wellness programs based on the group health needs in your area. It also helps us all become better stewards of our limited financial resources. Finally, if you complete both parts by August 31, 2015, you will earn the $300 credit in your HRA, if you are part of a Bon Secours medical plan.

6. Will my individual information be kept private?

Your individual information is completely confidential and will only be used as part of aggregate data to develop programs to improve your health and that of other employees. Your information will be protected under HIPAA.
You can be certain thatthe results of your PHA will have no impact on your employment status.

7. Why offer this?

The PHA is an important part of our “Healthy Me, Healthy You” wellness initiative designed to promote employee health and wellness. Knowing your numbers with the help of the PHA is a crucial first step to better health. In a climate of spiraling health care costs, good health is valuable to everyone. As health care providers, this is especially true for us. Good Help to Those in Need® is not just something we say, it’s our mission that relies on our good health and our good stewardship of resources.

8. Is every Bon Secours location doing the PHA?

Yes, this is a system-wide initiative.

9. Who will conduct the PHA for Bon Secours?

To ensure employee confidentiality Bon Secours has contracted with two independent companies. LSE will provide the health screenings and BSDI will provide the online wellness profile. Your individual health information is completely confidential and will not be shared with Bon Secours.

10. What information will Bon Secours receive?

While individual results are strictly confidential, our wellness contractors will collect and analyze the overall data and provide it to us as summarized information. That will help us to design responsive health and wellness programs targeting each Bon Secours location’s specific needs. For example, we may learn that a large number of employees face highstress levels. This might lead us to allocate resources for work/life balance offerings or evaluate organizational factors. Your results will not impact your medical premium, eligibility or employee contribution.

11. Will there be a follow up to the PHA to help me?

Your confidential PHA results will also enable our independent wellness contractors and our Health Plan partners to target helpful resources just for you. This will take place without the involvement of any Bon Secoursstaff,so that your individual results remain confidential.

12. What are the benefits of doing a PHA?

There are many benefits of doing the PHA—for you, your family, Bon Secours, and for our patients.

For you and your family:

  • Know your numbers. This is a case where knowledge is definitely power. Knowing your key health indicators will empower you to set goals for better health.
  • Get educated. Your participation will connect you with a range of targeted resources, making it easier for you to make healthier choices.
  • Get healthy. The PHA is a concrete step toward getting healthier—for yourself and your family.
  • Get accountable. The PHA will help you be accountable to yourself for your health.
  • Address existing or emerging health problems. By benchmarking key indicators now, you can make changes to improve, prevent or better manage a variety of health conditions.
  • Save money.Getting healthier can help you save money on medical costs. In addition, by completing both parts of the PHA by August 31, you will earn a $300 wellnesscredit in your HRA (see below).

  • Allocate resources. Be better stewards of our health and wellness dollars, allocating resources to those areas where help is most needed.
  • Plan wisely.Develop strategic programs to support employees’ health needs on a highly targeted basis, and to help plan more effectively for future needs.
  • Reduce medical costs. By empowering employees to make better health choices, we are doing our part to reduce sharply escalating health care costs associated with chronic diseases and other medical conditions.Many chronic and severe health conditions—including heart disease, diabetes and many others—can be improved, prevented or even reversed by good health choices.
  • Fulfill our mission. Our mission relies on sound financial stewardship and excellent health care.

For our patients:

  • Better care. Healthy caregivers are able to provide patients with better care.

13. I’m worried about what I might learn about my health.

When it comes to your health, ignorance is not bliss. Even if you learn that you have a health problem, getting the facts is a first step to change. Even small changes can add up to better health over time. 14. What was that about a credit in my HRA?

By completing both parts of the PHA by August 31, 2015, you will qualify for a $300wellness credit in a Health Reimbursement Account. The credit is only available to those enrolled in a Bon Secours Medical Plan.
