Instructions
1.The employee downloads the performance evaluation form from the online performance evaluation system.
2.The employee fills in their self-assessment and sets up a meeting with their supervisor(s) to discuss their performance in the past year based on the set objectives. Other areas to be discussed are achievements, challenges faced and areas that need development.The supervisor and staff also agree upon a set of objectives and training needs for the period ahead.
3.The employee uploads the updated form in the performance evaluation system.
4.The 1st supervisor logs into the performance management system and keys in the supervisor rating, comments and submits the form to HR or the 2nd supervisor where applicable.
5.The 2nd supervisor logs into the system and views the completed performance evaluation form and keys in the rating, supervisor comments and submits to HR.
6.Performance Evaluation Committee meets to receive reports from HR and to review and moderate PE ratings and thereafter the Senior Management Committee meets to approve the results of the performance evaluation exercise
GUIDELINES TO Performance rating (only round numbers are acceptable)
5=All aspects of performance of exceptional quality, consistently produces work that receives acclaim both within and outside the Centre.
4=Consistently meets and frequently exceeds job requirements both in terms of quality and quantity. Widespread peer approval and support both within and outside of the Centre as applicable.
3=Fully meets expectations of work programme at a satisfactory level of quality and quantity.
2=Agreed work programme not completed or of poor quality. Frequent criticism of performance and work output is evident.
1 = Usually does not meet job requirements. Work is consistently of poor work quality and output is very low.
