Chagrin Falls Physical Education Student Safety Contract

GENERAL RULES

  1. Conduct yourself in a responsible manner at all times in the Fitness Center.
  1. Athletes should not be in Center unless supervised by a staff member.
  1. No food or drink permitted in the Fitness Center (unless water).
  1. Never fool around in the Center. Horseplay,

practical jokes and pranks can be dangerous around the equipment.

  1. Make sure you are familiar with the locations of the safety exits, AED and fire alarms.
  1. Free weights/dumbbells should be used only on carpeted or rubber floor.
  1. Free weights and other equipment should be returned to their designated areas.
  1. Wipe down all equipment after each use. Paper towels and sanitizers are located around the Center.
  1. Notify physical education staff of any broken equipment or supply needs.
  1. Sound system should not be adjusted by athletes/students.
  1. Make sure all windows are closed and locked before leaving the Center.
  1. Backpacks and other personal items should not be brought into the Center.
  1. The Center is used by current students, coaches or faculty only.
  1. The Center is not responsible for lost or stolen items; please keep valuables in a safe place.
  1. Appropriate workout attire is required.
  1. Aerobic equipment should be limited to 30 minutes if others are waiting.
  1. If you use any functional training equipment (hurdles, ladders, etc.) make sure to return them to their appropriate area.

AGREEMENT

I, ______(student’s name) have read and agree to follow all of the safety rules set forth in this contract. I realize that I must obey these rules to ensure my own safety and that of my fellow classmates and instructors. I will cooperate to the fullest extent with my instructor and fellow students to maintain a safe environment. I am aware that any violation of this safety contract that results in unsafe conduct in the Fitness Center or misbehavior on my part may result in being removed from the Fitness Center, receiving a failing grade and/or dismissal from the course.

We feel that you should be informed regarding the school’s effort to create and maintain a safe environment in the Fitness Center. Your signature on this contract indicates that you have read this Fitness Center safety contract, are aware of the measures taken to ensure the safety of your son/daughter in the Fitness Center and will instruct your son/daughter to uphold his/her agreement to follow these rules and procedures for the Fitness Center.
