You are hereby given notice of the following disciplinary action:

¨ Disciplinary suspension without pay
¨ Discharge
This disciplinary notice is issued to you for unacceptable conduct/performance related to (check all that apply):
¨ Failing to abide by the lawful rules and regulations posted by the College;
¨ Failing to obey, or failing to abide by, a lawful order or instruction given by duly authorized College authorities;
¨ Conviction of a felony (or acts, which if prosecuted, would constitute such), when the nature or cause of which act would substantially interfere with the performance of his or duties at the college or when continued employment would cause the College substantial loss of confidence by/from the citizens of the District;
¨ Serious or repeated misconduct which causes the College to lose confidence in the employee’s fitness to continue as an employee. Such conduct may include, without being limited to, the following: violence or threats of violence; dishonesty or theft; failure to observe safety or quality guidelines; harassing or intimidating behaviors; or neglect of duty;
¨ Failure to carry out his/her official duties at an appropriate level of quantity and/or quality;
¨ Conduct the nature of which may disrupt or substantially interfere with an
ongoing administrative process of the college.
Briefly summarize the essential facts upon which this discipline is based. Identify the specific work rules, performance or conduct standards violated.
Briefly describe the specific impact of the employee’s performance/conduct on departmental or college operations.
If applicable, summarize the employee’s previous discipline record, if the previous discipline is relevant to the “theme” of the current discipline or demonstrates a pattern of misconduct.
Identify any specific behavior or performance requirements expected of the employee in the future. Specify either quantifiable or observable expectations and a time frame in which they are to be achieved. These plans of improvement shall be of sufficient length to allow the employee to demonstrate actual improvement. Employees and managers are jointly responsible for monitoring and follow-up related to expectations.
Outline the potential consequences of failure to comply with the expectations.
It is management’s intent to provide notification of this action to the IACE Professional Relations Officer. Such notice shall not include details related to the facts or circumstances surrounding the disciplinary event. You have the right to elect that such notification not be made, if you so choose.
A copy of this written warning will be placed in your personnel file. Any recurrence of these behaviors will be dealt with by further disciplinary action up to and including termination as provided by Article 15 of the IACE contract.
