This post is available from as soon as possible until 1st August 2017.The Directorate provides Renal Services for Greater Glasgow & Clyde and Forth Valley Health Boards and Renal Transplant Services for the West of Scotland. Inpatient services are located at the New Queen Elizabeth University Hospital Glasgow. The post will offer excellent training and experience in renal transplant surgery and nephrology. Previous surgical experience and FRCS is preferable but not essential.

This post holds educational approval from the Postgraduate Dean. If appointed as a Clinical Fellow the post will not be recognised for training.

To practice medicine in the UK you need to hold both GMC registration and a licence to practise.

1.Brief Description of the Health Board

NHS Greater Glasgow and Clyde is the largest group of adult acute hospitals in Scotland – offering many opportunities to ensure job satisfaction and career development. It provides a wide range of services from community-based care through to supra-regional services including cardiac and renal transplantation.

It enjoys close links with Glasgow’s three Universities and makes a significant contribution to teaching at both undergraduate and postgraduate level. Research also has a high profile within the organisation.

Renal services are included within the Regional Services Directorate which manages the following services

2.Hospitals and Clinics where Services are to be provided

The Renal Transplant Unit at the NewQueenElizabethUniversityHospital is the sole provider of Renal Transplantation for the adult population of the West of Scotland (population approximately 2.7 million). This Service performs more than 150 renal transplants a year, has one of the largest Living Donor programmes in the UK and supports the Scottish Paediatric Transplant Programme which is based at The Royal Hospital for Children. The Department also provides vascular access surgery for the 600 patients on haemodialysis, access surgery for peritoneal dialysis as well as general surgery for patients with renal failure.

The Renal Transplant Unit is now located on the Fourth Floor of the new adult hospital, co-located with the vascular surgery unit, that also houses a suite of Operating theatres and an Intensive Therapy Unit. All inpatient nephrology services for Glasgow, Clyde and ForthValley now occupy the remainder of the fourth floor. Outpatient haemodialysis activity for the QEUHG and the peritoneal dialysis unit are on the second floor.

Most outpatient haemodialysis occurs outside the inpatient facility with units situated at NewVictoriaHospital, Glasgow Royal Infirmary, NewStobhillHospital, InverclydeHospital, Vale of Leven Hospital and LarbertHospital. Carried out at satellite haemodialysis units at NewStobhillHospital, NewVictoriaHospital, FalkirkDistrictHospital, InverclydeHospital, and Vale of Leven Hospital.

Paediatric renal transplants are performed at the NewRoyalHospital for Children working in association with the paediatric surgeons based there.

All renal outpatient clinics are conducted in the new Victoria and StobhillHospitals which have excellent modern facilities including day case surgery suites which are used for renal (access) surgery in all appropriate patients.

There is a close working relationship between the transplantation programme and the University Department of Surgery where excellent laboratory facilities exit for research related to all aspects of organ transplantation. There is also close collaboration between the vascular surgery service and StrathclydeUniversity.

3.The Work of the Directorate/Department/Specialty

There has been an expansion in renal transplant activity over the last decade and a total of over 2000 transplants have been carried out. More than 150 cadaveric, living related and unrelated donor operations are performed annually with approximately 25-30% being from live donors. The current waiting list for renal transplantation in the West of Scotland is over 300. There are approximately 50 CAPD patients and 600 haemodialysis patients being treated within Greater Glasgow and Clyde and Forth Valley on an outpatient basis. In addition to the local catchment population, the renal unit at the QEUH also provides more complicated dialysis access surgery for other renal units and functions as a specialist referral centre. The unit is therefore one of the largest in the country.

There are 6 Consultant Transplant Surgeons and the renal surgical junior staff posts consist of one Specialist Registrar, 2 x Clinical Fellows, and three surgical trainees STs/FTSTAs. Joint physician/surgeon ward rounds are conducted daily. There are also regular Renal Unit research, audit, mortality/morbidity, pathology and radiology meetings.There is an emergency theatre 24 hours a day which is available for emergency and unscheduled work including transplants.

As well as being one of the largest transplant programmes in the country with opportunities for clinical research, excellent facilities exist within the University Department of Surgery. There are established links for experimental research in the fields of transplantation immunology and cellular biology at StrathclydeUniversity. We have several projects and post-graduates involved in understanding the vascular changes associated with renal failure with both cellular and small animal studies.

5.Duties of the Post

Clinical Commitments:

The appointee will participate with his/her middle grade colleagues in the pre-operative assessment and preparation, surgical management and post operative care of patients with renal failure. This includes attendance at outpatient clinics, ward rounds, a day surgery list as well as elective and emergency operating sessions. This clinical activity will account for an average of 2 sessions a week. There is a 1 in 6 on call commitment. The unit does not have Foundation Year doctors and all the routine ward care of the patients is undertaken by Case Managers / Nurse Practitioners who refer to the middle grade doctors for diagnostic and treatment decisions.

Teaching:

During term time there is a regular commitment to the undergraduate medical course particularly in ward teaching.There is a commitment to the postgraduate education of the trainees within the renal department.

Clinical Research:

There is an active clinical research activity associated with this large and busy transplantation programme and the appointee will be expected to contribute to this.

Laboratory Research:

Laboratory research is ongoing in collaboration with the University of Glasgow. There is the opportunity to gain laboratory experience in addition to clinically supporting this work.

Timetable:

The appointee will work with the trainees to provide middle grade cover for the transplant and renal surgery service. This will involve consultant run operating sessions, both inpatient and day surgery, transplant follow up clinics and assessment and renal surgery clinics. The appointee will be involved in the management of the inpatient care of the renal patients.

The on call will be a 1 in 6 rota (with prospective cover). Whilst on call for renal surgery the appointee will provide middle grade cover for emergency vascular and peritoneal dialysis at the QEUH, advice and if necessary attendance at the NVSHs as well as for kidney implantation and associated problems in the renal patients.

The middle grade posts within the department are currently graded as Band 2A and is reassessed on a regular basis and may be subject to change.

The appointee will also perform ward rounds and contribute to radiology, pathology, mortality, research and other departmental meetings within the combined nephrology and renal surgery multi-disciplinary structure.

TERMS AND CONDITIONS OF SERVICE

The conditions of service are those laid down and amended from time to time by the Hospital and Medical & Dental Whitley Council.

- If appointed as an StR (Locum Appointment for Training) the salary range will be: £30,911 to £48,605 per annum (pro rata)
- If appointed as a Clinical Fellow the salary range will be: £30,911 to £40,896 per annum (pro rata)
New Entrants to the NHS will normally commence on the minimum point of the salary scale, (dependent on qualifications and experience). Salary is paid monthly by Bank Credit Transfer.
HOURS OF DUTY / Full Time40.00
SUPERANNUATION / New entrants to NHS Greater Glasgow and Clyde who are aged sixteen but under seventy five will be enrolled automatically into membership of the NHS Pension Scheme. Should you choose to "opt out" arrangements can be made to do this via:
