It is a requirementthat the documentation of the individual making the PVG application is checked by a Scottish Gymnastics approved person i.e. CPC or Head coach (having attended the PVG workshop) inthe presence of the individual. The “ID Checker” (verifier) should validate the identity by: * Checking the individual’s identity against the required forms of evidence. *Making sure you tick the box of each form of ID shown to you.

Important: Please note that due to the high number of mistakes being made on PVG application forms, therefore delaying the processes by some considerable amount of time, the verifier shouldplease check that the following boxes have been completed BEFORE posting the application forms into Scottish Gymnastics;

For Application to join the PVG Schemethe following boxes should be completed:

A-1, 2 and 3.

B-1,2,3,5,13,14,15,16,17,18,19, 21, 23,25,28,31,36,38,40,42, 44/45.

C-1 Sign and date the application

D-1 and D3 (if the answer to D1 is no).

E-1, 2, 3 (should be Scottish Gymnastics), 5 (should be either coach, helper, chaperone etc.).

For Existing PVG Scheme Member Applicationthe following boxes should be completed:

A-1 (should be scheme record update), 2 (should be children), 3(should be yes), 4 (should be no)

B-1 (this number is on the original PVG certificate under your date of birth), 2,3,4,6,7and 8(should be no)

C-1 Sign and date the application

D-1 and D3 (if the answer to D1 is no)

E-1 (should be no), 2 (should be Scottish Gymnastics), 4 (should be either coach, helper, chaperone etc.).

NO OTHER BOXES SHOULD BE COMPLETED APART FROM THOSE MENTIONED ABOVE

Sample PVG application forms, ID verification and guidance forms as well as all other paperwork and information you will need you can be found on the SGA website. Go to clubs> click on safeguarding for clubs, a drop down box will appear containing various polices, procedures, templates download etc. Click on recruitment and PVG to get access to the sample PVG forms and guidance documents.
