PERSONAL HISTORY PACKET INSTRUCTIONS
The following instructions are furnished as a guide to assist you in filling out the Personal History Packet. These forms, which you are required to fill out, must be complete and detailed in all respects. It is the basis of your background investigation, which will be conducted to determine your qualifications for the position you have applied for with the Western Shoshone Department of Public Safety.
All questions must be answered completely and accurately. If they do not apply to you, indicate with N/A. Falsification or failure to include information, as directed, will be considered just grounds for non-acceptance or termination, if already employed. Avoid errors by reading the directions carefully before making any entries on the forms. Make sure your information is correct and in the proper sequence before you begin.
You are responsible for obtaining correct and complete addresses. If you are not sure of an address, check it either by personal verification or by correspondence. Your local library may have a directory service in addition to maintaining copies of all local telephone directories.
Copies of the following are required, if applicable, in order to complete the investigation:
Ø All of your educational transcripts/certificates
Ø Copy of your High School Diploma or GED
Ø Military service records/DD214 form
Ø Copy of your Birth Certificate
Ø Naturalization papers (if applicable)
Ø Copy of your Driver’s License
Ø Abstract of Driving Records (can be obtained from the Department of Licensing for a fee)
Ø Copies of Marriage License(s) and Divorce Decree(s) as applicable
Ø 2 Copies of your Fingerprints (these can be obtained at your local law enforcement agency)
Whenever describing a specific situation, make sure you give all the facts pertaining to it. Present the information in such a manner that any person, whether familiar or unfamiliar with the situation, will be provided with all the details and facts in the order in which they occurred, the dates or times the events took place, and the names of the persons or organizations involved. Please feel free to use additional paper as needed.
This form has been designed to encourage, rather than discourage, applicants for the Western Shoshone Department of Public Safety. It has been designed to allow you to present your qualifications in the most positive manner. Any information, which might be important, can and should be explained so that the person(s) reviewing your packet can more adequately understand your position.
Please note that all waivers at the front of the packet must be signed in the presence of a notary public. If you are mailing your packet, it is recommended that you send it via certified mail with a return receipt request.
