Special Conditions:

1.  Amendment No. 3 (12/22/2011) This Contract Amendment No. 3 incorporates the original award and is subject to the Standard Terms and Conditions contained in the original IFB 03707 and hereby allows the contract to be extended for a period of twenty-four (24) months from 1/4/2012 through 1/3/2014. All other terms and conditions remain unchanged.
