Registration Packet Checklist

This checklist will assist you in preparing the required documentation for your child prior to the orientation meeting on. Please fill out all of these documents, sign, date and return completed paperwork to: Turlock Nursery School or Kim Jorritsma21670 BlossAve. Hilmar CA 95324

  2. Admission Agreement
  3. Liability, Insurance and Photo Release Form
  4. Identification and Emergency Information Form
  5. Personal Rights Form
  6. Physician’s Report –Child Care Centers (Please note: This document is completed by your child’s doctor. Please be sure to allow ample time for completion by the deadline. If your child is not due for a physical prior to school starting, you must provide this form based on their physical completed within the past 12 months.)
  7. Child’s Preadmission Health History – Parent’s Report
  8. Consent for Emergency Medical Treatment
  9. Emergency Contact Form
  10. Handbook Signature Page (at the end of updated 2017-2018 Handbook)

All of these forms are available on our website, They can be found under Waitlist and Registration Forms, on the right side, under Individual Registration Packet. There will not be hard copies sent to you.

You must also include:

  1. TB Verification Form (This applies to all parents/family members who plan to volunteer at TNS).
  2. Copy of child’s current immunization record/card. As of January 1, 2016, state law requires that students must have received all immunizations prior to entering school. There are no longer any exemptions given.

Please note that we combine the days of the first and last months of Aug & June (this is the standard monthly tuition posted) in your first month’s tuition and that will be collected at the Orientation meeting. We look forward to seeing you at orientation (time and date TBA)

Last Days of School: TTh – Thursday, June 7, 2018

MWF – Friday, June 8, 2018
