DIRECTIONS FOR APPLYING FOR AN
ALPHA EPSILON PHI FOUNDATION UNDERGRADUATE SCHOLARSHIP
1. Scholarships are only available to rising juniors and seniors.
2. All applications must be typed or legibly printed.
3. A minimum 3.0 g.p.a. is required for consideration for a scholarship.
4. Scholarship recipients must be full-time students, in the US, for the entire 2013-1014 school year.
5. Enclose a letter specifying:
A. Your activities in your chapter, on campus and in the general community.
B. What you have done to supplement your parents' contribution toward your education. Please include specific employment information.
C. Your need for scholarship consideration.
6. Send the COMPLETED application, and letter postmarked no later than April 1, 2013 to:

7. Include a recent photograph with your application.
8. Also please have the following sent to Melissa Siegel no later than April 1, 2013:
A. Two letters of Recommendations
1) From your chapter advisor or a member of your Chapter Support Team
2) From a professor or employer;
(Letters from undergraduate members or from a family member will not be accepted)
B. An official university grade transcript.
C. Optional: Copies of signed 2012 Federal Income Tax Forms 1040 for both parents and applicant (first two pages only). All information will be kept in the strictest of confidence. Please contact Melissa Siegel if your 2012 tax forms are not available.
Scholarship recipients will be announced in June at Convention.
Payments will not be made until proof of registration has been submitted as directed.
Scholarships are not available for study abroad.
