- Membership form must be submitted unless you are selecting the non-member rate or you have already paid for your new membership effective October 1st 2016! Questions? Contact Registration Team at ndicate CAPED Membership in the subject line of your email.
- Please complete one form per person. Complete the three pageform; make copies of the form as needed. Incomplete registrations will not be processed.
- Full payment by check, credit card, or purchase order must accompany registration form. MAKE CHECKS PAYABLE to: “CAPED Convention 2016”
- NO REFUNDS WILL BE GIVEN AFTER 09/21/16. No Exceptions.
