CMC VELLORE EQAS – TRANSFUSION MEDICINE MODULE

IMPORTANT INFORMATION

Dear Participant,

We are dispatching theinformation regardingrenewal of registration to participate inCMC Vellore External QualityAssessment Scheme-Transfusion Medicine Module for the year 2018.

There are some important changes:

1.The folder sent to you will now contain only the covering letter and subscription details. All other documentation has been uploaded onto the website: from where the Manual, Registration form and Subscription details can be down loaded. If you have problems with this please send us an e-mail.

2.You will have to renew your registration with payment of subscription fees every year.

3.When you return the Registration form and the payment in the form of a demand draft, please make sure that your current PIN NUMBER (4 digits starting with 1###) is clearly printed on the front page.

4.In all your correspondence remember to indicate your PIN Number.

5.In case there is any change in the name of the doctor in-charge of the blood bank/laboratory, address, land line / mobile number and e-mail ID, inform us immediately.

6.We have had issues with the Courier package not being accepted at the designated address. Kindly instruct your staff concerned to receive the survey samples and results even if you are out of station / country since this is for the institution.

7.Before sending the survey sample/ our analysis report by courier, each time we are sending out an e-mail. If the survey samples are not received within the time mentioned, please get in touch with us. If no communication is received, we will assume that it had been received by you.

8.If you are not in receipt of survey samples/our analysis report before the next survey samples, please get in touch with us immediately indicating the PIN Number. If not we will assume that it has reached you.

9.The participant must return results in all the three surveys in order to qualify for the Participation Certificate.

10.Please keep all papers relative to this programme in the file that we have sent.

11.Requests received after one week from the date of our communication, for samples /duplicate analysis report, may not be honoured.

12.Please share this information with your colleague and the staff in-charge of the laboratory.
