Knowledge Sharing Inventory – Due November 30, 2013

Objective: The Knowledge Sharing Inventory allows us to keep track of what information is being collected by our partners to prevent duplication of monitoring efforts and to determine where we can add to existing monitoring tools. Keeping this table up to date allows us to save time and resources 

Instructions: Please complete the table bellow outlining what information is being collected by your PRIMARY partners. Stick with monitoring that corresponds with N@C’s key intervention areas. The first row provides an example. Please add rows as necessary.

MOH / Report from the subdistrict health officers on coverage of service delivery including:

/ District Health Officers/Civil Surgeon / Electronic report sent via email / Monthly / There are delays in three layers. First one is from Union level to Sub-district level. In the Subdistrict the AHI (Assistant health inspector) submit their report. Sometime they cannot submit the report in scheduled time. In the subdistrict level the Statistician give entry of the report and send both hard copy and soft copy to District Health Officer/Civil Surgeon. Due to delayed meeting in the Sub-district Statistician cannot enter or compile report properly. Sometimes there are poor internet connectivity for this they can submit the electronic report timely. There are also entry errors in the report.
