Reporting Information
The monthly reporting sheet and quarterly reporting form were created for your department’s use. The intention of these documents is to provide you with a fast, easy way to track your department’s public education efforts and for ease of reporting for ISO bonus points. Please note: the monthly reporting sheet is provided to you as an internal reporting document only. However, please send in the quarterly reporting form and any public education photos (including descriptions) to the NCFLSE State Council at the email address provided:
Below are some comments and tips for completing these sheets/forms:
1)Please complete all information at the top of the page.
2)Record the number of programs that your department conducts:
Example 1: Go to a school and show the fire truck to 6 classes separately from 0900 to 1230. When you record that data, you did 6 programs.
Example 2: Visit the “Lunch Plus” program at a local church to talk to seniors about CO alarms. Do one presentation for all attendees from 1130-1215. At the end of the period, three adults ask you about other safety devices. You would record this as one program. Each individual or question does not constitute an additional program.
3)Columns and rows will automatically total.
4)Types of Presentations:
- Public Relations: Any type of PR event such as a “Meet and Greet” where one serves as a departmental representative. Meetings, Media events, etc. that involve your participation with jurisdiction’s Public Relations personnel or media contacts. Mascots fall into this category. No formal safety lessons are involved.
- Residential Survey: Many departments are now conducting residential surveys, residential home inspections or safety checks. This is where these types of programs are recorded. Foster cares inspections could be recorded here, as well.
- Safety Device Install: smoke alarms, hearing impaired smoke alarms/bed shakers, CO alarms, residential Knox Boxes, house numbers, and other safety devices such as tub rails, night lights, etc.
- Special Event: Parade, Safety Fair, Open House, etc.
- Other: Any program not covered in the program topics listed.
5)Presented to:
- Civic Group: Rotary Club, The Pilot Club, Jr. Charity League, Ruitan Club, and other service organizations.
- Individual: For use with residential survey, installation of safety devices, etc.
- Religious group: religious institutions and organizations. AWANA, Methodist Men, etc.
- Special Interest group: Homeowners’ Associations, political groups, ladies organizations, etc. Specifically for adults.
- Target Hazard: Per ISO, this is a category to record. Worded in ISO as “Large Loss Potential or Hazardous Condition.” Hospitals, Large Residential, Government Housing, Lumber Yard, Prison, Chemical facilities, etc. For a detailed listing of target hazards in your area, speak to someone in your Inspections department.
- Youth Group: Scout packs/troops, Ball leagues, Fire & Life Safety Bowl teams, etc. Specifically for children and teens.
- Other: Any group not covered in the participants listed.
6)Program Saves: Please take a few moments to record any significant saves or reduction of injuries in response to a specific program. Additionally, please complete the “SAVES” report on the OSFM website to help track saves throughout N.C.
Example 1: After your smoke alarm program in December, seven families install smoke alarms throughout their homes. In January, one of those families escapes a fire because of the sounding smoke alarms. This is recorded as a SAVE here and on the OSFM website.
Example 2: After a school-wide bicycle helmet program, you notice an increased usage of helmets and safety pads in your community. This would be something to report on the NC FLSE form.
7)Other comments: Please take a few moments to share your top five programs or any success stories your department has had as a result of your public education efforts. This could include grants awarded, media coverage, acknowledgement from jurisdiction, etc.
8)This is not listed on the sheet or form, however, feel free to share your personal or departmental success with others:
Throughout the year, feel free to submit any personal or departmental recognition you receive as a result of the public education programs you conduct. This recognition could be media coverage, awards, certifications, letters from the community, etc. NCFLSE will distribute this information to Fire & Life Safety Education groups across the state.
