2016/17 REGISTRATION PACKAGE


In order to register your child with Frog Lake Minor Hockey Association (FLMHA) BOTH PARENTS/GUARDIANSwill need to have completed the Respect in Sport for Parents program. You will be asked to provide proof of completion. FLMHA WILL NOT be covering this expense ($12.35) for either parentany further.

Incomplete registration forms will not be accepted and will be returned to registrant until all information is complete. This includes: payment, bond cheques, birth certificates and Player declaration/verification forms.

PAYMENT OPTIONS

  1. All fees must be paid by August10th by cash (receipt w/date), post-dated check or Bank draft for August10th. For Payment options contact the Treasurer

  • NSF cheques immediately VOID the player’s registration and the registration process
  1. Two new fees have been added to the registration package this season; Alberta Native Provincials Preparation Fee, Alberta Treaties Hockey Championships Preparation Fee, these are to ensure that there is a commitment to play with Frog Lake teams during these events.

*NSF cheques immediately VOID the player’s registration and the registration process will need to be completed again with a $35.00 NSF fee and late registration charges being applied if applicable.

PLAYER REGISTRATION POLICY

  • EARLY BIRD DEADLINE TO REGISTER IS July 15/REGULAR DEADLINE TO REGISTER IS August 10
  • Players must reside within FLMHA boundaries. Any players residing outside FLMHA boundaries are subject to FLMHA approval and appropriate provincial association approval.
  • Initiation players must be 5 years old by Dec 31st of the current registration year.
  • In the event that team rosters are too full at August 10th registration deadline, players will need to try out for the team. If they do not make the team, a full refund will be issued or players will be released to other communities
  • A $50 late fee will apply after August 10th. FLMHA reserves the right to accept your registration based on availability within the age appropriate division.
  • Late fee does not apply to first time players in FLMHA and imports
  • A waitlist will be created for those who register after August 10th, but are not guaranteed a spot on a team to play.

Any required releases, concessions, or other documentation must be submitted to FLMHA before the player’s first ice-time, and is the responsibility of the player. FLMHA will, to the best of their ability, try and ensure that players are aware of all required documentation at the time of registration. However, it is ultimately the parent’s responsibility to read registration policies and complete any required documents. Please contact the FLMHA

REQUIRED FORMS

Can be found online at

  1. Color Copy of Birth Certificates must be enclosed when registering for the first time and when registering for the first time.
  2. Player Verification Form – To be completed by players who reside outside FLMHA boundaries. (Alberta residents only)
  3. Parent Declaration Form – For players who have moved into FLMHA boundaries. Hockey Alberta requires that this form must be accompanied by two additional pieces of documentation to verify the address information. This documentation can consist of:
  4. A proof of sale document for a house or home with the family’s name and address.
  5. A letter from a school indicating that the player is registered with the school.
  6. A utility bill, with the family’s name and address.
  7. Any piece of Government issued document that contains the family’s name and address (e.g. Driver’s License, Tax Assessment, etc.)
  8. Permission to Try out Form – Any player who resides outside FLMHA boundaries is considered a non-resident player or import and must complete this required form before participating in any try-outs.

SPECIAL EVENTS POLICY

Fundraising Activities

Each team MUST fundraise throughout the season, all parent(s)/guardian(s) of registered players must participate in fundraising activities to benefit from the teams fundraising goals. All parent(s)/guardian(s) of registered players must communicate with team coaching staff to ensure all items are understood and agreed to.

Jersey Security Deposit

Each parent(s) or guardian(s) must agree and sign a Jersey sign-out consent form to declare their responsibility of Jersey throughout season asper registered player. If unable to produce proof that the jersey is in your possession, YOU are responsible to make arrangements for the replacement.

NOTE:

There will be a formal letter of notice that there will be an association suspensionfor if playerFAILS TO SHOW JERSEY at game after requested by Manager and will not participate in games until returned or the complete amount of a replacement is provided.

Please submit $100 Security Deposit with each player’s registration.

Conditions and Waivers

Frog Lake Minor Hockey Association Waiver:

By signing this document, I agree to abide by the rules and regulations, and decisions and all duly approved amendments thereto of Hockey Canada, it’s Board of Directors, its Branches and /or divisions which may be restrictive in some areas such as movement from Team to Team, conduct ect.. Further the information requested above is required by Hockey Alberta to facilitate hockey programs on behalf of the registrant and Hockey Canada. Hockey Canada will treat this personal information with the upmost respect in accordance with the Hockey Canada Privacy Policy at all times. All registrants are subject to review by the Registrar and no registration will be approved until all fees are paid and required documentation is received. Registration is undertaken with the understanding that additional team fees and or fundraising may be required depending on the level of play.

Further I agree to abide by the terms and conditions of the Parent’s code of conduct. I also agree to ensure the jersey deposit tothe FLMHA in the sum of $100.00.

I hereby consent to the participation of the above player in the Frog Lake Minor Hockey Association program. I agree to and do hereby indemnify and save harmless and release the FLMHA, its officers, directors, coaches, Manager’s, representatives and insurers (collectively referred to as “FLMHA”) from any and all claims, actions, costs, expenses and demands of whatsoever kind, in respect to loss, damage, bodily injury or death to persons, including the player, or to property, which may arise out of or in connection with the player’s participation in the program, unless such loss is occasioned by or attributable solely to the gross negligence of the FLMHA. I acknowledge that the FLMHA hereby reserves the right to request that the player withdraw from the program prior to its conclusion if, in the opinion of the Executive, the player is not acting in a responsible manner or displaying appropriate conduct, or in the event that the player’s behavior is deemed to constitute a danger to the health, safety or well being of any other player.

Hockey Canada Waiver:

I, the undersigned certify the information to be provided to be true and in consideration of the granting of this registration to me with the privileges incident thereto, and by registering I have become subject to the rules, regulations and decisions of Hockey Canada, its Board of Directors, its Branches and/or divisions which may be restrictive in some areas such as movement from team to team, conduct etc. and I agree to abide by such rules, regulations and decisions of Hockey Canada, its Board of Directors, its Branches and/or divisions. Further, the information to be provided is required by Hockey Canada to facilitate hockey programs on behalf of the registrant and Hockey Canada. Hockey Canada does not sell, trade or otherwise share the information we collect outside our Branches and Associations however we may from time to time use this information for the purposes of offering additional services, promotions, including promotions offered by third parties, and/or hockey specific research. FREEDOM OF INFORMATION AND PROTECTION OF PRIVACY NOTICE AND CONSENT

The purpose of this notice and consent is to provide documentation on the use of player information, pictures, and statistics for non-profit purposes and to obtain consent for such use.

  1. Player information is collected and maintained so as to properly coordinate and operate the Frog Lake Minor Hockey Association program. It is also provided to Hockey Alberta, Hockey Canada, or any league of the player’s team, for purposes of registration, recording, statistical information, and insurance.
  1. Player's names, information, data, photos, and comments may also be used in team or league newsletters, annual reports, Frog Lake Minor Hockey Association and Hockey Alberta web sites, radio, newspaper, and other hockey or local publications.
  1. Individual and team photos may be taken and displayed in the local newspaper, local arenas, and the office of the Frog Lake Minor Hockey Association. Other reports and advertisements prepared by the Frog Lake Minor Hockey Association are permitted as well.
  1. Players’ names and parent/guardian telephone, e-mail, and other information may be used for the purposes of team, league, and Frog Lake Minor Hockey Association communication matters.
  1. Player's names may be included on lists for the purposes of team placement and tracking player statistics by team, league, Frog Lake Minor Hockey Association, and Hockey Alberta, and such statistics may be displayed on their websites.
  1. This consent shall remain active for as long as the player is a member of the Frog Lake Minor Hockey Association and need only be signed once.

ZERO TOLERANCE POLICY AND PARENT/GUARDIAN PLEDGE

Frog Lake Minor Hockey Association now has a Zero Tolerance Policy. Any verbal, emotional and/orphysical abuse or harassment towards players, officials, volunteers and fans will not be tolerated. Offenders will be instructed to leave the premises immediately.

This policy will be enforced jointly with the RCMP.

Parent/Guardian Pledge

This pledge is in accordance with the afore mentioned ZERO TOLERANCE POLICY.

1. I will support and work with my child to fulfill his/her commitment.

2. I will ensure that my child plays for his/her enjoyment. (Not mine)

3. I will encourage my child to play by the rules and display proper sportsmanship.

4. I will applaud and acknowledge great efforts & good plays by my child and his opponents.

5. I will not question any officials judgment publicly.

6. I will support all efforts to remove verbal and physical abuse from children’s hockey.

7. I will respect and show appreciation to volunteers who devote their time for my child to play hockey.

The Frog Lake Minor Hockey Association believe strongly in the values of teamwork, sportsmanship and leadership through example. As a player in the Frog Lake First Nations community, your actions reflect not only on you, but also on your teammates, your coaches and the Frog Lake Hockey team.

Frog Lake Minor Hockey Player Code of Conduct Agreement

As a parent/legal guardian of a child involved in a Frog Lake Minor Hockey team, I agree to abide by and follow the rules of the guidelines below.

  • I will never curse or use foul language.
  • I will respect my coaches at all times and listen carefully to their instructions. I understand that they are giving their time to make me a better hockey player. This includes bus drivers and the buses that are supplied on away games. We must keep buses clean!
  • I will support and respect my teammates and opponents at all times. I will not fight, tease, ridicule, put down or make fun of any players on my own team or the opposing team, at any time.
  • I understand that my behavior reflects on my teammates, my community and the Frog Lake Minor Hockey team. I will always be on my best behavior at games, practices, hotels and other team events.
  • I understand that my parents/legal guardians must not enter the Frog Lake Minor Hockey team dressing room once the League game is underway, unless otherwise asked to by the Frog Lake Minor Hockey team Coaching Staff or in the case of an emergency.
  • I will not complain about penalties or calls made by the officials and I will respect the referees at all times. I understand that they are in charge of enforcing the rules of ice hockey, and I will respect their opinions at all times.
  • I will show good sportsmanship at all times, both winning and losing. I will always congratulate the other team and wish them good luck.
  • I will arrive at practices and games on time and prepared. I understand that it is my responsibility to have all necessary equipment needed to play.
  • I will always leave the Dressing rooms clean and in good condition.
  • I will never bring Drugs, Alcohol or Weapons of any kind to games, practices, trips or other events.
  • I give permission to the school that my child attends to share and communicate with the Frog Lake T-Birds Minor Hockey Association regarding my child’s behavior and attendance.
  • All points apply to any current Social Media (i.e. facebook, instagram…), team related websites and/or any form of internet related forms of communication – Cyber Bullying in all forms is not tolerated.

I understand by signing this agreement I promise to uphold the Frog Lake Minor Hockey Player Code of Conduct. I realize that violation of this Code of Conduct may result in loss of ice-time, suspension, or even ejection from the Frog Lake Minor Hockey team.

Code of Ethics:

1)All Frog Lake Minor Hockey players MUST be in school. The local schools have been asked to provide verification.

2)All Home Games – players MUST arrive at the Frog Lake New Horizons Centre least one hour prior to game.

3)It is mandatory that parents (or a responsible adult) must stay during their child(ren)’s practises.

4)You cannot store your hockey equipment at the Frog Lake New Horizons Centre.

5)Curfew is at 9:00pm for players 16 years and under without parent/guardian supervision.

6)Final Rosters in by December 1 with no new additions possible.

7)All Protective Gear must be worn by Frog Lake Minor Hockey Players (Neckguards & Cages...)

8)By December 1st of the Hockey Season, each hockey team in the province of Alberta must have at least two registered team officials (coaches) be on the bench with certification in their Respect in Sport “Activity Leader/Coach” program or at least have the equivalent “Speak Out” program certification already completed as a condition of their eligibility to coach.

9)By December 1st of Hockey Season, completion of the Respect in Sport “Parent Program” for each family of every player shall a condition of their eligibility to participate on a sanctioned hockey team. (excluding Jr. & Sr. Divisions.)

10)By December 1st of the Hockey Season, each official (referee and linesman) over the age of 18 be certified in the Respect in Sport “Activity Leader/Coach” program or at least have the equivalent “Speak Out” program certification already completed as a condition of their eligibility to officiate.

11)Dressing Rooms must be kept clean after each practise and home games. (Hockey tape, Broken Sticks and miscellaneous trash...)

12)Parents are required to volunteer on Frog Lake Minor Hockey Home Games. (Time keeping, Cleaning of rooms, 50/50 draw and fundraisers)
