Asset Audit Exception Form

This form is to be used for purposes of documenting asset information and stewardship when an asset is:

·  Located off of UC campus

·  Unable to be viewed since it is an intangible asset such as software

·  Unable to be viewed since it is installed inside another asset

·  Unable to be viewed due to its size/weight/immobility

·  Unable to be viewed since it is in a location not physically accessible by an auditor

·  Unable to be tagged with an Asset Inventory Tag due to the nature of the asset such as:

o  Software license, internal component, insufficient surface area or unusual contour of surface does not allow for permanent adhesion, environmental conditions such as high temperature may cause destruction of tag or interference with asset's calibration and/or operation.

This form should be completed and provided to the UC Asset Auditor at the time of a capital asset audit. Whenever any of the original data changes (i.e. asset location, individual responsible, etc.), AND/OR when preparing for a capital asset audit, a new form must be completed with current data.

Note: If the asset location changes, the location on the Asset Master Record in UCFlex must also be updated.
