A WisconsinDOT-issueddriverlicense,evenifexpired,revoked orsuspended.

A WisconsinDOT-issuedidentificationcard(available forfree,see below)

MilitaryIDcard issued bya U.S.uniformedservice;includingretiredmilitary. (may have an “indefinite” expiration date)

A U.S.passportbookorcard

Thefollowingforms ofproofofidentificationarealsoacceptableforvotingpurposes,butmustbeunexpired:

A certificateofnaturalizationthatwasissuednotearlierthantwo yearsbeforethedateofan electionatwhichitispresented

A VeteransAffairsID Card(may have no expiration date)

A driverlicensereceiptissued byWisconsinDOT(valid for45 days)

Anidentificationcardreceiptissued byWisconsinDOT (validfor45 days)

A citation of Notice to Revoke or Suspend WI Driver License (valid for 60 days)

An ID Petition Process receipt issued by Wisconsin DOT (valid for 180 days)*

Thefollowingformofproofofidentification isacceptableforvotingpurposes,and may beexpired:

Aphotoidentificationcardissued bya Wisconsinaccrediteduniversityorcollegethatcontainsthefollowing:

  • Date ofissuance
  • Signature ofStudent
  • Expirationdateno laterthan two yearsafterdateofissuance
  • TheuniversityorcollegeID mustbe accompanied bya separatedocumentthatprovesenrollment

Thefollowingformofproofofidentification isacceptableforvotingpurposesregardlessoftheexpiration date:

Anidentificationcardissued bya federallyrecognizedIndiantribein Wisconsin

Anacceptableproofofidentificationmustbereceivedbythe municipalclerkby(insert by-mailabsenteerequestdeadline), andcan beprovided bymail, fax, oremail. Ifyouare unabletosubmitproofofidentification bythisdeadline,youwillnotbe sentan absenteeballotforthiselection. Toensureyourabsenteeballotissentinatimelymannerpleasesubmitanacceptableform ofproofofidentification assoon aspossible.
