Dear Staff Applicant,
Thank you for your interest in serving Christ at Cedarbrook Camp of Ohio this summer. You will see God at work in the lives of campers as well as in your own life. We welcome you! The next step in the application process is a background check required by both the state of Ohio and our accrediting agency, the American Camp Association.
The easiest way to complete a fingerprint background check is through a webcheck site in Ohio. If you plan to be in Ohio anytime up to 2 weeks before serving at camp– please plan on getting this done at that time. If not – follow the directions below for manual ink fingerprints.
Enclosed are three items that need to be completed: a BCI (Ohio Bureau of Criminal Investigation) card, an FBI card and a Request for Exemption from Electronic Fingerprint Submission Requirement form. Out-of-state applicants will need to complete and submit the forms by June, 16, 2017 in order to allow time for processing prior to camp. Some information on the cards has been completed for you.
Please follow the instructions carefully:
•All information must be entered in black ink. Please print clearly as illegible or incomplete cards will not be processed. Complete the cards before going to the law enforcement agency.
•You will need to contact a law enforcement agency in your area regarding getting fingerprints done. Some agencies will do it, others won’t. Some charge a small fee, others do it free. Some take cash, others don’t. Some require an appointment, others don’t. Call first!
•The fee for the Ohio BCI check is $22.00. The fee for the FBI check is $24.00. After your fingerprints are done, mail cards, the Request for Exemption form and your checks in the envelope provided. Payment can be made by personal check, money order or certified check. BCI requests that you make two separate checks. Both checks should be made out to: Treasurer, State of Ohio.
•Out of state employees who are hired will be reimbursed $10.00 in your Tuck shop account for each of the fingerprint cards completed (a total of $20.00). We realize this does not cover your cost completely, but the Cedarbrook Board wanted to mitigate your cost in this way.
•Take your payment, driver’s license or state ID, both of the completed fingerprint cards and the completed Request for Exemption from Electronic Fingerprint Submission Requirement form with you when you go to the law enforcement agency to be fingerprinted.
•A BCI addressed envelope is included in this packet. Sometimes the law enforcement agency will send the fingerprint cards to BCI. More often they will tell you to send the cards, Request for Exemption form and payment (2 checks/money orders/certified checks) to BCI.
