Willits Frontier Days INC. Release of Liability

All contestants must sign this liability waiver and if under 18, (age is considered as of January 1, 2015 a parent or guardian must also sign). Any form not properly signed will be rejected. Any team event will be considered as one contestant but each member must sign the waiver & if applicable, signed by a parent or guardian.

I acknowledge that horseback riding is a sport which carries inherent risk of injury and damage to me, my horse and property. I hereby enter the Willits Frontier Days Ranch Rodeo at my own risk and subject to all rules and regulations of the rodeo. I further agree that if any damage be occasion by, or loss, or damage occur from any cause to the horse or myself, I will make no claims thereof. I further agree to indemnify Willits Frontier Days, INC and the City of Willits against any legal proceeding for personal injuries or property or property damage arising out of my participation in this event. I agree to the conditions set forth.

Mail entries to: Willits Frontier Days, Po Box 800, Willits Ca. 95490. Make checks payable to: Willits Frontier Days INC. A $25 fee will be charged on all non-sufficient fund (NSF) checks.

Drawing out: After entry deadline, entry fees will only be refunded upon receipt of a medical statement due to injury or illness and only to member of team affected.
