External Rx History
External Rx History is a tool that can assist with obtaining an accurate medication list. This tool provides access to SureScripts which is a database that lists prescriptions filled by patients in a provided timeframe. We can only view information from pharmacies that participate with SureScripts.
1. Click External Rx History.
2. Select either View or Import.
a. View brings up a window that lists the External Rx History.
b. Import brings up a side by side comparison of the External med Hx in the left and Current Medication List on the right. This is the best option.
3. When the Rx History Patient Consent window displays, click Consent Granted. While this is obtained on the facesheet during the registration process, it needs to be acknowledged electronically . ¥ Note: This window will only display once per patient per facility and the wording is specific to the facility.
Comparing Documented Home Meds in Med List with External Rx History
Note: If the patient’s External Rx History has been retrieved in the past 3 days, refresh will not be available and the date/time stamp will display the last time External Rx History was retrieved. This only queries every 3 days.
Tips for more accurate med lists
ü If you are presented with an order sentence window, select an appropriate option. Note: Do not select Convert existing SIG if present.
ü Medications that convert as Med Not Found/Not Known/ NonFormulary should not be added as a home medication whenever better options exist. Instead, use the +Add button to add the medication to the Medication Hx.
ü The information provided via SureScripts may not contain all of the order information or may not be the way the patient takes the medication. Always verify the information with the patient and add any variance under the compliance section.
ü Remove any diplicate medications that may be present.
