JOB TITLE:Referrals and Assessment Co-ordinator Part Time

HOURS: 20hours per week(TBC within operational hours Mon-Fri 9am-5pm)

LOCATION: Dudley

REPORTS TO: Director of Therapeutic Services

RESPONSIBLE FOR:Assessments for individuals presenting for treatment and provision of referral information to individuals who call/email the treatment centre.

OVERALL JOB PURPOSE:

  • To work closely with the Referrals and Assessment Co-ordinator, reporting to the Director of Therapeutic Services, to process all referrals for the Gordon Moody Association.
  • Communicate with all applicants, arranging initial interviews and processing application paperwork. Conducting interviews as and when appropriate.
  • Co ordinate and process pre treatment support to include up to date records of support received and issues arising from that.
  • Liaising with referring agencies and other professionals prior to entry to assist with obtaining all relevant information needed to process applications.
  • Assist with travel arrangements and any problems with accessing treatment
  • Update the central treatment data base (Apricot)

KEY RESPONSIBILITIES

The duties and responsibilities of the Referrals and AssessmentCo-ordinatorcan be broadly divided into the following activities:

KEY TASKS:

  • Respond to enquiries, referrals and direct applications which may arrive via website, email, phone, letter or personal visit, liaising with the other Referrals and Assessment Co-ordinator to ensure adequate and consistent cover.
  • Support to the Director of Therapeutic Services in assessing and communicating with applicants. To include liaising with referral agencies and significant individuals in relation to assessing suitability for treatment.
  • Collate and report information about the treatment programme and the enquiries received which will be used for research and monitoring purposes.
  • Keep accurate records re resident information. To include working with and adding to the data base containing resident information from application stage to entry.
  • To ensure all applications and referrals are prioritised, contacted, and assessed within agreed timescales.
  • Provide information and advice on their application and issue further information regarding the Gordon Moody treatment programme (to include sign posting to other agencies if applicable)
  • To provide up to date information regarding the costing for treatment, including eligibility for UK state and housing benefits.
  • Ensure the completion of theSafety, Need and Gambling Audits,in accordance with the GMA Treatment programme policies and procedures, with a view to assessing risk and suitability for assessment/treatment.
  • To ensure that all applicants and referring agencies are advised of the outcome of applications and referrals to the service.
  • To work within the referrals team to allocate individuals to appropriate GMA Centres.
  • To provide a fast, professional and sensitive response providing timely and accurate information to anyone who contacts Gordon Moody Association.
  • To respond to enquiries to the service, signpost and refer people to a range of services and organisations.
  • Ensure that relevant paperwork is completed and placed on file as per GMA policies and procedures.
  • Make use of communication technology to update the Apricot database and to monitor progress.
  • To cover main switchboard as and when required
