Tip Tap Toes Dance Studio Waiver 2017

Payment Disclaimer

I understand that I have entered into a payment obligation for the period in which I am enrolled in the dance program at Tip Tap Toes Dance Studio. I understand that my total yearly tuition has been divided into equal monthly payments that are due by the 1st of each month. If payment is not received by the 7th day of the calendar month, my account will be charged a $10 late fee. If a check is returned, my account will be charged $25. If tuition payment is not received in 60 days, participation at Tip Tap Toes Dance Studio will be jeopardized. Any accounts that have become 90 days past due will be referred to an attorney for collections. If I terminate my enrollment at Tip Tap Toes Dance Studio, I understand that I will not receive any kind of reimbursement of payments that Tip Tap Toes may have already received.

Each students account requires an active credit card number on file. If Tip Tap Toes Dance Studio has not received your monthly installment payment by the 10th of the month the credit card on file will be charged the installment amount plus the $10 late fee. When credit cards are within 2 months of their expiration we will contact you for an updated credit card.

If I choose, I may pay the tuition for the whole year up front or pay the remaining balance in full throughout the dance season. By doing so I understand that my tuition payment is non refundable.

If I am unable to continue dancing at Tip Tap Toes or must take a short or extended leave of absence due to any type of medical illness or injury, I am responsible for providing Tip Tap Toes Dance Studio with the proper medical note in order to receive any kind of refund.

Liability Disclaimer

Tip Tap Toes Dance Studio and/or its instructors are not liable for personal injuries occurring during class time, rehearsals, dance conventions, competitions or performance on and off the premises of TTT. Students and/or parents are individually responsible to inform the instructor or owner of any physical limitations which may prevent full participation in studio activities. I understand that it is my responsibility to inform Tip Tap Toes of any -changes to my registration form and medical form throughout the 2017-2018 dance season. Further, I grant Tip Tap Toes Dance Studio employees, instructors or agents permission to authorize any emergency medical treatment that may be required for my child.

I understand that Tip Tap Toes Dance Studio or its instructors are not liable for the loss of or damage to personal property.

Photo Consent Disclaimer

I give Tip Tap Toes Dance Studio consent to use my/my child’s likeness in a photograph in any and/or all of its publications which may include promotional flyers, website, Facebook page or any other promotional avenues without compensation or right to ownership.

Weather/Make Up Class Disclaimer

Tip Tap Toes Dance Studio will follow the Chesterfield County School System’s closing policies regarding holidays and inclement weather. Tip Tap Toes holds the right not to hold make up classes in situations such as this. Monthly tuitions are based on the yearly price of dance, not the total days of dancing in the calendar month. I understand that tuition will not be pro-rated in cases where inclement weather and/or holidays have interrupted the normal working hours of Tip Tap Toes Dance Studio.

______I understand that Tip Tap Toes Dance Studio is not responsible for anybody’s personal property or belongings.

______I understand that Tip Tap Toes Dance Studio does not offer make up classes or refunds for cancellations due to inclement weather, holidays, illness or any absence.

______I understand that monthly payments are the same each month, regardless of holidays, inclement weather or student absence.

______I understand that a $10 late fee will be enforced if tuition is not paid in full by the 7th of each month. I also understand that an unpaid account for 60 days will result in the students continuation of dance at TTT to be in jeopardy until the account is brought into good standing. An account in default by 90 days will be referred to an attorney for collections.

______I understand that there is a $25.00 fee for all checks that are returned (bounced) to Tip Tap Toes.

______I understand that accounts paid in full before the dance season or during the dance season are non-refundable.

______I understand that after 10 days of non-payment, my credit card will be charged the tuition amount plus a $10 late fee.

______I give Tip Tap Toes Dance Studio consent to use my child’s photograph for promotional flyers, website, Facebook page or any other promotional avenue without compensation or right to ownership.

______I have read and agree to the Tip Tap Toes Dance Studio Waiver, which includes Payment Disclaimer, Liability Disclaimer, Photo Consent Disclaimer and the Weather/Make Up Class Disclaimer.
