  1. INSPECT & VERIFY PRECODE MATERIALS

Inspect & Review precoded materials for accuracy.

Do NOT change ID, name or date of birth field on forms. If corrections are needed in these areas the precoded form must be voided and a new answer document must be created (hand bubbled). Keep a list of names and titles/departments of people involved and duties assigned. Remember to always have verification signatures by the staff member(s) responsible for verifying that precoded information is correct.

When your CTC receives the campus precode lists for each test administration, what staff members are responsible for verifying all of the information?

CTC’s will be notified as soon as test materials are delivered to Accountability/Assessment. CTC’s must pick up materials as soon as possible and will have 48 hours from pick up of secure material to check shipment to ensure it is complete. Any discrepancies must be reported to Office of Accountability/Assessment within 48 hours of receipt of material.

The CTC is ultimately responsible for making sure that all test materials are counted and documented on a Materials Control Form for each test administration.

Districts are required to maintain documentation for 5 years following a test administration. There are specific forms and documents that must be submitted to HCISD Office of Accountability / Assessment after each test administration for archive. Documentation retention must be in accordance with the district. Required Documentation Binder Checklist.

Campus Test Plan (Operational)

The following must be completed by the CTC and/or designee to submit to HCISD Office of Accountability / Assessment.

TRAINING

Attend Campus Test Coordinator (CTC) training

Training is required prior to campus training and receipt of test materials.

Read current District/Campus Coordinator Manual, Test Security Supplement, and Test Administrator Manuals.

Campus Training Dates – schedule test administrator training

Place your training dates on your campus calendar

Notify teachers formally of training schedule for the year

List training dates by administration and identify who will be required to attend the training.

Inform teachers how make-ups and absences will be handled on your campus.

  • Campus staff are required to receive annual training in Test Security and General Administration Testing procedures.
  • Who is responsible for checking the training sign-in sheets to ensure every teacher/staff member has attended training, received their test administrator manual (if applicable), and signed an oath?

Test Administrator Training:

Oaths must be completed and collected prior to the TAs handling secure materials – this can be done at the end of training or prior to testing

Oaths must also be signed when the week of testing is complete

A copy of all security oaths MUST be turned into Office of Accountability/Assessment as well as a copy of all sign in sheets.

TEST ADMINISTRATION

Front office staff must be provided lists of where students are testing, test administrators assignments, and hall/restroom monitor assignments in case of an emergency.

Prepare list of student testing assignments.

Students must know prior to the day of testing where they will be testing.

You must correctly identify students who are taking the test. If you are on a large campus and/or the test administrator does not personally know all of the students that will be testing, it is recommended that students present identification when signing in to ensure that the correct student is being tested.

Determine the number of test administrators, additional proctors, and alternates that will be needed.

  • It is better to train extras in case of emergencies. How many staff members will you train? Will you be using non-certified staff to assist with testing? In what capacity and under what circumstances?
  • Who will serve as restroom and hall monitors? How many will you need? Who will train them and when will that training take place?
  • Select the teachers that will serve as test administrators (consult with principal). Please attach the list to this document. List their name, grade/subject that they teach, and what grades/subjects they will be administering.
  • Create a campus policy for make-up training sessions for test administrators. Review it with your principal. How do you intend to notify teachers of the policy? How will you ensure that all teachers attend training or make-up training for each test administration? How will you deal with those who refuse to attend or those who never show? Explain your policy here.

TURN TEST MATERIALS TO DTC – OFFICE OF ACCOUNTABILITY / ASSESSMENT

Return all materials to Office of Accountability/Assessment at the date/time specified. Scorables may be returned first and non-scorables may be returned at a later date, as scorables ship before non-scorables.

  • Who will be responsible for verifying that all test booklets and secure materials are accounted for after testing has ended (before returning them to Office of Accountability/Assessment)?
  • Hand gridded answer documents must be together at the top of the stack of answer documents per grade level. This is to ensure that all students new to the district have all the necessary demographic information bubbled. This will be verified upon your delivery of scorable materials to Office of Accountability/Assessment.
  • Who on your campus will be responsible for checking and verifying that answer documents have been properly prepared for return to be scored? (Score codes, test taken info, and accommodations bubbled correctly; documents bundled correctly under appropriate header sheets).
  • Who will assemble all the required documentation that must be returned to Accountability/Assessment? (seating charts, signed oaths, material control forms, training sign in sheets, etc.)

Make arrangements for instruction, location, and supervision of non-testing students.

Seniors may be included in this arrangement.

ITEMS TO CONSIDER

The following may be used as a reference / guide to assist campuses with test decision making.

SELECT TEST SITES

  • Which classrooms will be used for testing?
  • Will the library or other rooms be needed for small group or individual test administrations?
  • What rooms will be used as overflow for students who are testing later in the day when most of the other students have completed testing? What procedures will you use for moving students and test material to the new location?

TEST SCHEDULES

  • What grades/subjects are testing on each specific day?
  • If you have a testing window, who will decide when to test during that window?
  • What is your plan for students who do not test on a specific day?
  • What is your plan for students who finish testing early?

ENVIRONMENT

Control access to test areas using staff members/monitors.

Post testing signs at the entrance to the campus informing visitors that testing is occurring.

Each testing room must also have a “Testing Do Not Disturb!” sign posted outside the room.

All testing room doors must remain unlocked during testing and windows to the doors must be completely uncovered for monitoring purposes.

All District Monitors that represent Central Office will have already been trained and will have signed their oath. They will be wearing their district ID badges.

Only staff members who have been trained and who have signed an oath may enter a location where secure test material is located and students are testing.

Cell Phone Policy: The use of cell phones or other electronic media devices is not permitted in the testing environment, nor are they allowed during breaks.

Computers and cell phones must be turned off in all testing rooms.

  • What is your campus plan for informing parents, students, and staff of the policy and how do you plan to enforce it during all state test administrations?
  • How will the CTC/Principal communicate with TAs (and vice-versa) during testing? (Describe your campus procedures)

LUNCH

Will you be serving lunch? YES NO

Procedures for lunch/snack as needed:

Tests are timed. Most students will not break for lunch during the test.

Students must not eat or drink at their desks/tables while test material is in their possession.

Students still testing must eat lunch as a group and must be supervised during lunch by a trained staff member to ensure they do not discuss the test.

If lunch is provided, they should be placed in a location away from the test material (a table at the back/side of the room, etc.). Answer documents should be placed inside test booklets, which should be closed, and left on the top of student desks. The TA should ensure that test material is not disturbed and must supervise students during lunch to ensure they are not discussing test information. Lunch will stop the clock.

PLANNING FOR ACCESSIBILITY WITHIN THE STAAR PROGRAM

Careful review of state accessibility policy is required so that students receive only those accessibility features and designated supports that are allowed or approved by TEA for use on state assessments. Testing coordinators, test administrators, and other campus personnel involved in state testing must consider the following questions related to ensuring accessibility for students on test day.
