Data Sharing AgreementWednesday, 06 April 2013
This agreement covers a data sharing relationship between parties

A copy of the agreement should be provided to Trust Office and the original copy will be held by the responsible organisation.
Nature of arrangement/partnership
IRIS Connect provides a data sharing agreement with SCHOOL 1 and the GDST under their respective End User Licence Agreements (EULAs). These EULAs establish IRIS Connect as designated data processors for both organisations. This supplementary agreement extends that relationship to allow <SCHOOL 1> and the GDST to share data via the IRIS Connect system under the conditions specified below.
PURPOSE – Teacher Professional Development/Training
Type(s) of data / personal data / Lesson observation videos, observer and observe names and observational data such as performance feedback coaching and coaching objectives
statistical / other data / Observational metadata generated via IRIS Connect observational proformas and counter and timer tools
Method(s) of transfer / shared access / Access will be provided via the IRIS Connect secure web – based learning platform via role based log in.
physical transfer / There will be no physical transfer of data
electronic transfer / Electronic transfer will be made over the web via a secure SSL connection
Security provisions / designated responsibilities / The schools will remain the data managers (with individual video permissions delegated to observed teachers) and IRIS Connect will act as data processors for the purposes of the Data Protection Act (DPA). Under their end user licence agreement the schoolshave obligations to use the data responsibly by keeping passwords secure and forbidding 3rd party access. Each observation will be individually permissioned via the scheduling process contained within the IRIS Connect system. IRIS Connect manages a log which shows, who requested the lesson observation and for what purpose, as well as the date and time at which the observee gave permission for this to happen and when the observation happened.
encryption / All data is subject to 128 bit encrypted once it enters the IRIS Connect system. Transfers are made via port 443 using SSL encryption and user authentication.
formats used / Formats of data storage include html based web data held on a secure server, the .asf, .movand .flv video formats for secure upload and streaming
Records created / Records will be held on the IRIS Connect system in the form of an observation library. Videos and associated meta data are made available to observer and observee with sharing and deletion rights delegated to the observee
Who has access to the data / Data is only made available to the observed teacher. The observed teacher may choose to share the video between users of the IRIS Connect System within the parties.
What will happen to the data / The observee, thepartiesdata controllershave control of the data.
We hereby confirm that the arrangements described above accurately represent the data shared and the methods and formats of transfer as well as and other recordkeeping arrangements required.
Where personal data is being shared, we also confirm that the information will be used for the purposes stated in this agreement and will not be shared with or passed to any third parties not already named in this agreement. When personal data is disposed of this will be done in such a way to ensure confidentiality is preserved.
Where personal data is being collected in order to be shared, data subjects will be informed of the purposes for which the data is being collected and to whom it will be passed.
