Forms submitted after August 1st will not be processed until the office re-opens in September rendering the student unable to participate until that time. If your child is unable to schedule an appointment with your private physician to meet the August 1st deadline, you may consider contacting one of the local medical walk-in centers that offer low cost sport physical exams.

The Nurse’s office is closed during the summer; however, the nurses will be in for flexible per diem hours to process sport forms. Sport forms submitted over the summer must be mailed or dropped off at the Athletic Office of the High School. Medical forms should not be given to the coach. Once submitted, all sport physical forms will be processed through the nurse’s office and then sent to the School Medical Director for review before the student is considered medically cleared for participation. The student’s name is then sent on to the athletic office for final clearance.

1.  Once a year, every sport candidate must submit a physical examination performed by his or her private health care provider and recorded on the approved NJ Department of Education Pre-Participation Physical Evaluation Packet. Only a health care provider who has completed the Student-Athlete Cardiac Assessment Professional Development Module is eligible to perform the physical exam. All areas on the 2-page, 2-sided Pre-Participation Physical Evaluation packet must completed. This includes the date of the physical exam and answers to all yes/no questions and the signatures of the student and the parent on page 1 (& page 2 if applicable), documentation of all physical assessment values and the signature of the examiner on pages 3 & 4. Incomplete forms will not be processed and will be returned to you for completion. An exam is valid for 365 days from the start of a given athletic season. If an approved, unexpired physical exam is already on file in the nurse’s office from a previous sport season you do not need to submit another copy.

2.  Each athletic season, every sport candidate whose medical examination was completed more than 90 days prior to the first practice session must complete a 90-Day Health History Update Questionnaire. This form must be completed and signed by the parent/guardian. (Do not date the form prior to June 1, 2015).

3.  Each athletic season, every sport candidate must submit the three forms indicating that you have read the information regarding Consent to Random Testing, the Concussion Policy Acknowledgment, and Sudden Cardiac Death in Young Athletes. These forms are required by NJSIAA and must be signed by both the student and parent/guardian and submitted, with the appropriate medical forms, to the nurse. Failure to sign these forms renders the student athlete ineligible for athletic participation.

4.  Each athletic season, every sport candidate must complete a Sport Participation Card. The student and the parent/guardian must sign this form.

Once the student is deemed medically eligible for participation, the Athletic Office is notified. Any questions regarding further clearance for participation should be directed to that office at 908-889-8600 ex 3414.

If you need to verify the date of you child’s last recorded physical, or if you have any questions regarding obtaining medical clearance, you may contact the nursing staff prior to June 18th during regular school hours at 889-8600 ex. 3508. It is recommended that you keep a copy of your child’s physical exam for your personal files.
